Morphological signs of magnetic resonance urography in children with obstructive or complex urinary tract anomalies

Abstract

Introduction: In this study, we aimed to evaluate the morphological findings of children with obstructed and complicated urinary anomalies with magnetic resonance urography (MRU). Materials and Methods: 35 children (bilateral in 3, totally 38 renal units), who presented to our clinic between 2002 and 2005, were evaluated with traditional imaging techniques. Subsequently, MRU was performed for morphological evaluation. Results: After the evaluation of 17 cases diagnosed as ureteropelvic junction obstruction (UPJO) and 2 cases diagnosed as ureterovesical junction obstruction (UVBO) with T1-sequence MRU, a crossing aberrant vessel that may explain the etiology of the obstruction or any additional information about the etiology of the obstruction could not be provided in any of the patients. In 4 patients with ectopic ureteral insertion, the ureteral insertion that could not be determined with conventional imaging techniques and cystoscopy was visualized very clearly and demonstratively with MRU. In 12 cases with ureteral duplication (4 together with ureterocele) the anatomy and insertion sites of the ureters were demonstrated with MRU. Conclusion: MRU is an outstanding method since it provides good visualization, prevents the radiation exposure, enables the evaluation of the anatomy of collecting system, parenchyma and major renal vessels simultaneously in the morphological evaluation of urinary system (UrS) in new born, infants and children. But, anesthesia requirement and its high cost prevent the usage of MRU in obstructive pathologies in routine clinical practice. Nevertheless, it is the most appropriate method to determine especially the ectopic ureteral insertion anatomically in children with complicated urinary system pathology and to designate the surgery technique to be applied.

Özet

Obstrükte ve komplike üriner bozukluğu olan morfolojik çocukların bulgularını magnetik rezonans ürografi (MRÜ) ile araştırdık. 2002 ve 2005 yılları arasında kliniğimize başvuran eden 35 hasta (3'ü iki taraflı, toplam 38 böbrek birimi) önce geleneksel görüntüleme yöntemleri ile değerlendirildi. Sonra morfolojik inceleme için MR ürografi uygulandı. Üreteropelvik bileşke obstrüksiyonu (UPBO) tanısı konulan 17 olgu ve üreterovezikal bileşke obtrüksiyonu (UVBO) tanısı konulan 2 olgunun T1 sekans MRÜ ile değerlendirmesi sonrası, hastaların hiçbirinde obstrüksiyonun etiyolojisini açıklayabilecek çaprazlayan bir aberan damar veya obstrüksiyon etiyolojisi ile ilgili ek bir bilgi sağlamadı. Ektopik üreteral insersiyonlu 4 hastada konvansiyonel görüntüleme yöntemleri ve sistoskopi ile belirlenemeyen üreteral insersiyon MRÜ ile çok net ve açık bir şekilde belirlendi. Üreteral duplikasyonlu 12 olguda (4 hastada üreteroselle birlikte) olguda MRÜ ile üreterlerin anatomisi ve insersiyon yerleri belirlendi. MRÜ yenidoğan, infant ve çocuklarda üriner sistem (ÜS)'in morfolojik değerlendirilmesinde yüksek kalitede görüntü sağlama, radyasyona maruz kalmama, toplayıcı sistem, parankim, önemli böbrek damarlarının anatomisinin incelemesini aynı anda mümkün kılması nedenleriyle seçkin bir yöntemdir. Buna karşın anestezi gerektirmesi ve yüksek maliyeti MRÜ'nin obstrüktif patolojilerde rutin klinik uygulamadaki kullanımını engellemektedir. Bununla birlikte, komplike ÜS patolojili çocuklarda özellikle ektopik üreteral insersiyonu anatomik olarak ortaya koymada ve uygulanacak cerrahi tekniği belirlemede en uygun yöntemdir.
