The evaluation of biochemical risk factors for recurrent urinary stone disease

Abstract

Introduction: It is established that any urinary stone that receives no medical treatment, recurs in 50% of the patients and especially affects the young generation, causing a considerable morbidity and loss of working hours. We aimed to investigate the differences between biochemical profiles of the normal and stoneproducing populations, in order to determine the risk groups that require a metabolic evaluation and the extent of that evaluation. Materials and Methods: Our study groups consist of 118 patients, 76 males and 42 females, who were referred to our department between January 2000 to June 2003 for their recurrent and/or bilateral urinary stone disease and 53 others with no urinary stone disease history. Each subject was evaluated for serum biochemistry and stone composition and calcium, oxalate, citrate, uric acid, sodium, potassium, magnesium, ascorbic acid, protein levels, and pH values in 24-hour collected urine. Results: The most frequent metabolic abnormality in these stone formers was hypocitraturia and even the controls were found to have severe hypocitraturia (<320 mg/day). These patients had higher urinary levels of calcium, uric acid, oxalate, sodium and ascorbic acid, when compared to the controls. Furthermore, their fasting glycemia, blood cholesterol and triglyceride levels were higher. According to the stone analysis, calcium oxalate stones were the most frequent, and magnesium ammonium phosphate stones constituted 24% of all stones. Conclusion: Our findings suggest that males with urinary stone disease who have high fasting glycemia, blood cholesterol, triglyceride and creatinine values have a higher tendency for recurrence of stones and they should be evaluated in terms of hypocitraturia, hypercalciuria, hyperoxaluria, hyperuricosuria. This approach may eliminate the need for detailed, time-consuming, expensive tests.

Özellikle genç yaştaki hastalarda ciddi morbidite ve iş gücü kaybına neden olan bir üriner sistem taşının herhangi bir tıbbi tedavi almaması durumunda %50 oranında tekrarladığı bilinmektedir. Biz çalışmamızda, tekrarlayan veya iki taraflı üriner sistem taş hastalığı olanlar ile sağlam olguların biyokimyasal profillerini değerlendirip, hangi hastalar için metabolik araştırma gereklidir sorusuna cevap bulabilmeyi amaçladık. Bu nedenle 76 erkek, 42 kadın toplam 118 taşlı hasta ile 53 sağlam olgu serum biyokimyası ve 24 saatlik idrardaki değerleri açısından karşılaştırıldı. Taş hastalığının, özellikle genç yaştaki erkeklerde daha sık görüldüğü, en sık görülen metabolik anormalliğin hipositratüri olduğu ve bu hastaların sağlam olgulara göre 24 saatlik idrarlarında daha yüksek oranda ürik asit, kalsiyum, oksalat, sodyum ve askorbik asit bulunduğu tespit edildi. Serum biyokimyaları karşılaştırıldığında, taş hastalarının açlık glisemisi, kolesterol ve trigliserid değerlerinin sağlam olgulara göre daha fazla olduğu görüldü. Sonuç olarak, taş hastalığına sahip, erkek olgulardan rutin biyokimyasal analizlerinde yüksek açlık serum glisemi, kolesterol, trigliserid ve kreatinin değerlerine sahip ve idrarlarında proteinürisi olanlar, hastalığın tekrarlaması açısından riskli olarak değerlendirilmeli ve 24 saatlik idrarda en sık görülen, basit ve ucuz olarak tespit edilebilen hipositratüri, hiperkalsiüri, hiperoksalüri ve hiperürikozüri açısından araştırılmalıdır. Ancak bu şekilde ayrıntılı, zaman alıcı ve pahalı araştırma ve tetkiklerin uygulanmasına gerek olmayabilecektir.
