Our treatment results in adult patients wıth Wilms’ tumor

Abstract

Introduction: Although Wilms’ tumor (WT) is the most common renal tumor in children, in adults it is exceptionally rare. We reviewed our experience regarding adult patients with WT to assess their clinical characteristics and treatment guidelines according to stage. Materials and Methods: A total of 4 adult patients were enrolled in this study between 1998 and 2004. Clinical data were obtained by reviewing patient charts. In preoperative period all patients were evaluated with physical examination, abdominal ultrasonography (USG), abdomino-pelvic computerized tomography (CT), chest-x ray or CT scan of the chest. The patients were staged according to the National Wilms’ Tumor Study Group 4 (NWTS) staging system. Results: The median patient age at the time of diagnosis was 21.3 years. There were two patients with stage II disease and two patients with stage III disease. None of the patients had stage I disease. All patients underwent radical nephrectomy. The lymph node sampling was performed in all patients. Two patients with stage II disease received adjuvant chemotheraphy. These patients received a two drug regimen and no radiotheraphy and two patients with stage III disease received a three drug chemotheraphy regimen and radiotheraphy to the hemiabdomen. There was no recurrence and progression in a 28 months follow-up period. All tumors had favorable histology. Conclusion: Compared with children, adults with WT are reported to have a worse prognosis. It usually cannot be differentiated from other renal masses preoperatively. Because of the rarity of this disease, adults with WT are at a risk of either undertreatment or incorrect treatment. We believe that early diagnosis and treatment can improve the prognosis of adult WT.

Wilms tümörü (WT) çocuklarda en sık görülen böbrek tümörü olmasına karşın erişkinlerde oldukça ender görülür. Bu çalışmada erişkin WT’nin klinik özelliklerini ve evresine göre tedavi protokolü değerlendirdik. Bu çalışmaya 1998 ve 2004 yılları arasında 4 erişkin WT olan hasta alındı. Ameliyat öncesi tüm hastalara fiziksel inceleme, abdominal ultrasonografi (USG), abdomino-pelvik bilgisayarlı tomografi (BT), akciğer grafisi ve/veya toraks CT yapıldı. Hastaların evrelendirilmesi National Wilms Tumor Study-4 (NWTS) evrelendirme sistemine göre yapıldı. Hastaların teşhis konduğu andaki ortalama yaşları 21,3 idi. İki hastada evre II, diğer 2 hasta ise evre III hastalık mevcuttu. Hiçbir hasta evre I değildi. Tüm hastalara radikal nefrektomi yapıldı ve hastalardan lenf nodu örneklemesi çıkarıldı. Evre II olan 2 hastaya ikili kemoterapi verildi. Evre III olan 2 hastaya ise üçlü kemoterapi ve radyoterapi verildi. Hastaların hiçbirinde 28 aylık takip sürecinde bir ilerleme veya nüks gözlenmedi. Tümörlerin hepsi iyi bir histolojiye sahipti. Erişkin WT’si, çocuklarla karşılaştırıldığında hastalık seyri olarak daha kötü olduğu bildirilmiştir. Genellikle ameliyat öncesi diğer kitlelerden ayırt edilemez. Halen kesin bir tedavi protokolü yoktur. Çocukluk çağı WT hastalarındaki gibi uygun tedavi, erken teşhis ile erişkin hastalarda da WT’de önemli ilerlemeler kaydedileceğine inanmaktayız.
