A new technique: Tubeless percutaneous nephrolithotomy

Abstract
Introduction: Percutaneous nephrolithotomy (PNL) has recently replaced open stone surgery for the management of large kidney and upper ureteral stone. The last step in completion of PNL is the placement of a nephrostomy tube in order to provide hemostasis along the tract, avoid urinary extravasation and provide adequate drainage of the kidney. Recently there is no consensus regarding drainage of the kidney after percutaneous nephrolithotomy. The placement of a nephrostomy tube after PNL has been challenged and a new ‘tubeless technique’ has been introduced recently. In this review, the technique of tubeless PNL, types of urinary diversion and the results will be discussed with the light of the literature.
Özet
Günümüzde perkütan nefrolitotomi (PNL) büyük böbrek ve üst kısım üreter taşlarının tedavisinde önemli ölçüde açık cerrahinin yerini almıştır. PNL ameliyatının son basamağında klasik olarak, trakt boyunca hemostazı sağlamak, üriner ekstravazasyonu engellemek ve böbreğin yeterli drenajını sağlamak amaçlı nefrostomi kateteri yerleştirilmektedir. Perkütan nefrolitotomi ameliyatı sonunda nefrostomi kateterinin yerleştirilmesi standart bir yaklaşım olmasına karşın günümüzde PNL sonrası drenajı sağlamak için kesin bir konsensus bulunmamaktadır. Son yıllarda PNL sonrası nefrostomi kateterinin yerleştirilmesi sorgulanmakta ve tüpsüz PNL tekniği gündeme getirilmektedir. Bu derlemede tüpsüz PNL tekniği, çeşitleri, endikasyonları ve sonuçları yayınlar eşliğinde tartışılacaktır.
