Y-chromosome microdeletions in the azoospermia factor gene in fertile and idiopathic infertile men

Abstract

Introduction: In this study, the relation of AZF microdeletions in Y chromosome with the hormone and sperm parameters was investigated in severe male infertility. Materials and Methods: The study was carried out at Urology and Genetics departments of Medical Faculty of Ataturk University. Fifty infertile men as the patient group and 10 fertile men as the control group were included into the study. Testes volumes of the patients were determined by Prader orchidometer. Serum free testosterone (fT), total testosterone (tT), prolactin (PRL), follicle stimulating hormone (FSH), luteinizing hormone (LH) and estradiol (E2) levels were measured in the hormone laboratory. The genetic investigation was done with polimerase chain reaction (PCR) method at the Department of Genetics. The semen samples were examined under light microscope according to WHO criteria. The subjects were divided into four groups in terms of sperm parameters. The first group comprised azoospermic (n=30), the second group severe oligospermic (1-5 million/mL sperm n=10) and the third group mild oligospermic (5-15 million/mL sperm n=10) patients. Ten normospermic men were included as control group. Statistical analiysis of the parameters were done with Many-Whitney U, Kruskal Wallis and chi-square tests. Results: Microdeletion on Y chromosome was detected in 7 of 60 cases (6 from azoospermic group and 1 from severe oligospermic group). No microdeletion in Y chromosome was detected in the fertile and mild oligospermic groups. The deletion rate was 11.66% in all cases, 14% in the infertile group, 20% in the azoospermic group and 10% in severe oligospermic group. There were significant relations between AZF positivity with sperm count and FSH levels. Microdeletion rate increased as the sperm number decreased (p=0.018). There was significant correlation between serum FSH levels and AZF microdeletion positivity (p=0.001). There was not any significant relationship between the presence of AZF and testes volumes, serum E2, LH, PRL and T levels. Conclusion: We suggest that AZF microdeletions should be studied in azoospermic and severe oligoasthenozoospermic patients.

Bu çalışmamızda idiyopatik infertiliteli erkek hastaların semen analizleri, hormon değerleri, testis volümleri ile Y kromozomu üzerindeki AZF bölgesinde görülen mikrodelesyonlar arasındaki ilişkiyi incelemeyi amaçladık. Çalışmamıza kliniğimize infertilite nedeni ile başvuran 50 hasta ile kontrol grubu olarak aldığımız 10 fertil hasta alındı. Hastalar sperm ölçütlerine göre 4 gruba ayrıldı. Birinci grubu 30 azoospermik hasta oluştururken ikinci ve üçüncü grubu sırasıyla 10 ağır oligozoospermik ve 10 hafif oligozoospermik hasta oluşturdu. Dördüncü grup ise 10 fertil hastadan oluşturuldu. Hastaların hepsinin serum total testosteron (tT), serbest testeron (sT), prolaktin (PRL), folikül stimülan hormon (FSH), lüteinizan hormon (LH) ve estradiol (E2) değerleri ile testis volümleri değerlendirildi. Ayrıca bütün hastalara karyotip tayini ve Y kromozomundaki AZF bölgesinde mikrodelesyon değerlendirilmesi yapıldı. Altmış hastanın 7’sinde mikrodelesyon tespit edildi. Bu delesyonların 6’sı azoospermik, 1’i oligozoospermik grupta idi. Fertil grupta ve hafif oligozoospermik grupta mikrodelesyon tespit edilmedi. AZF pozitifliği ile sperm sayısı (p=0.018) ve FSH düzeyleri (p=0.001) arasında istatistiksel olarak anlamlı ilişki saptanırken, AZF varlığı halinde testis volümleri, E2, FSH, LH, PRL, tT ve sT düzeyleri arasında anlamlı ilişki tespit edilmedi (p>0.05).
