Unilateral hydronephrosis secondry to giant bladder stone

Abstract
Introduction: Bladder calculi account for 5% of urinary calculi and usually occur because of bladder outlet obstruction. Neurogenic voiding dysfunction, infection, foreign bodies, males with prostate disease or relevant surgery and women who undergo anti incontinence surgery are at a higher risk for developing bladder calculi. We present a case of with unilateral hydronephrosis seconder to giant bladder stone. A 55- year-old man was hospitalized with the chief complaints of lower abdominal pain, recurrent urinary infections, pollakisuria and with symptoms of benign prostate hyperplasia. Radiographs showed a giant bladder stone Cystolithotomy was performed under the diagnosis of bladder stone. The extirpated stone weighed 455 g, and measured 9.0x6.5x5.5 cm in size. The stone was composed of magnesium ammonium phosphate. The postoperative course was uneventful and the unilateral hydronephrosis improved markedly. This case report illustrates the importance of radiological evaluation of patients presenting with recurrent urinary infections and benign prostatic hyperplasia.
Özet
Mesane taşları tüm üriner sistem taş hastalıkların %5’inden sorumludur. Hazırlayıcı etkenler olarak genellikle, mesane çıkım obstrüksiyonları, nörojenik mesane, enfeksiyon, yada mesanede yabancı cisim sorumlu tutulmaktadır. Ayrıca erkeklerde benin prostat hiperplazisi (BPH), bayanlarda anti inkontinans cerrahisi mesane taşı gelişiminde risk etkenlerini oluşturmaktadır. Biz dev mesane taşına bağlı tek taraflı üreterohidronefrozlu bir olguyu sunmaya çalıştık. Kliniğimize 55 yaşında erkek hasta, başlıca alt batın bölgesinde ağrı, tekrarlayan üriner sistem enfeksiyonu, sık idrara çıkma, geceleri sık idrara kalkma ve idrar yaparken zorlanma şikayetiyle başvurdu. Yapılan tetkikler de özellikle çekilen direkt üriner sistem grafisinde (DUS) pelvik bölgede dev bir opasitenin olduğu gözlendi. Yapılan sistolitotomide alınan taşın ağırlığı 455 gr ağırlığında, boyutları 9.0x6.5x5.5 cm idi. Bu olgu bize tekrarlayan idrar yolu enfeksiyonu ve BPH şikayetiyle baş vuran hastalarda basit bir direkt üriner sistem grafisinin ne denli önemli olduğunu göstermektedir.
