The situation of RPLND in stage I nonsemınomatous testis cancer and value of risk factors in prediction of outcomes

Abstract
Introduction: We evaluated the outcome of primary retroperitoneal lymph node dissection (RPLND) and value of risk factors in patients with stage I nonseminomatous testis cancer (NSTC) at our department. Materials and Methods: The charts of patients with stage I NSTC who underwent primary RPLND were reviewed. Embrional carcinoma (EC) ratios and vascular invasion (VI) finding in orchiectomy specimens were correlated with histopathologic outcome of RPLND. Antegrade ejaculation status of the patients who underwent nerve sparing RPLND was found out. Results: Forty four of the 51 patients with stage I NSTC underwent primary RPLND whereas 7 of them elected conservative follow-up. Mean age at diagnosis and follow-up time were 23.15 (17-35) and 28 (9-84) months respectively. Sixteen cases had at least 1 risk factor. Left and right modified nerve sparing, and full template RPLNDs were performed in 13, 18 and 13 patients respectively. Retroperitoneal lymph nodes were positive in 14 cases, of whom 11 were stage N1 and 3 were N2. Retroperitoneal metastases were present in 5 of the 10 patients with VI, 7 of the 11 patients with EC ratio more than 50%, and in all 5 patients who had both risk factors. Twenty nine of the 31 patients who underwent NS RPLNDs had antegrade ejaculations within follow-up time. All patients were disease free at the end of the follow-up, except for 1 with distant metastasis who received chemotherapy and died on the 56th month of follow-up. Only 2 of the 7 patients who were conservatively managed at the beginning were available for follow-up for more than 1 year (Mean follow-up for this group: 20.1 months). Of these, 1 patient, who had one risk factor, received chemotherapy for a retroperitoneal mass. Conclusion: Primary RPLND in patients with stage I NSTC revealed retroperitoneal metastases in 32% of the cases. EC ratio more than 50% in orchiectomy specimen was a more valuable risk factor for predicting pN1 and pN2 status than VI in our series. Conservative management for these patients was not successful in our circumstances, whereas RPLND in stage I NSTC for both low and high risk patients proved effective and had few complications.

Bu çalışmamızda, evre 1 seminom dışı germ hücreli testis tümörlerinde (SDGHT) primer tedavi olarak retroperitoneal lenfadenektomi (RPLND) başarısını ve risk etkenlerinin değerini incelemeyi amaçladık. 1994-2007 yılları arasında evre 1 ve SDGHT olup ilk tedavi olarak RPLND uyguladığımız olgulara ait bulgular ve izlem sonuçları retrospektif olarak incelendi. Olguların testis patolojilerindeki embriyonal karsinom (EK) oranları ve vasküler invazyon (Vİ) varlığı incelendi ve RPLND patolojileri ile karşılaştırıldı. Evre 1 SDGHT’li 51 olgudan 44’üne ilk tedavi olarak RPLND uygulandı. 7 olgu izlem protokolünü seçti. Tanı anında yaş ortalaması 23,15 idi (17-35). Olguların 27’si sağ, 18’i sol tarafı tutan testis tümörü idi. İzlem süreleri ortalama 28 (9-84) ay idi. 16 olguda en az bir risk etkeni vardı, 28 olguda yoktu. 44 olgudan 13’üne sol 18’ine de sağ modifiye sinir koruyucu, 13’üne sinir korumaksızın tam template RPLND uygulandı. Toplam 14 olgunun retroperitonunda lenf nodu saptandı, bunların 11’i N1 ve 3’ü N2 idi. Vİ olan 10 olgunun 5’inde (3 ünde N1, 2’sinde N2), EK oranı %50’den fazla olan 11 olgunun 7’sinde (5 N1, 2 N2), her iki risk etkeni birlikte olan 5 olgunun hepsinde (3’ünde N1, 2’sinde N2) retroperitonda metastaz saptandı. RPLND’lerin 31’inde sinirler korundu ve bunların 29’unda izlem süresi içerisinde antegrad ejakülasyonun varlığı saptandı. Takibe alınan 7 hastadan yalnızca 2’si 1 yıldan fazla süreyle izlenebildiler. Hiçbir olguda izlem süresince retroperitonda metastaz saptanmadı. Evre 1 SDGHT olan hastalarda ilk tedavi olarak RPLND uygulandığında %32 oranında retroperitonda metastaz saptandı. Serimizde primer tümördeki EK oranının %50’den fazla oluşu vasküler invazyona kıyasla pN1-pN2’yi öngörmede daha etkili bulundu. İki risk etkeni birlikteliğinde %100 metastaz saptandı. Kendi hasta grubumuzda izlemin başarılı uygulanamadığını, gerek düşük, gerekse yüksek riskli evre 1 seminom dışı hastalıkta RPLND’nin etkin ve düşük istenmeyen yan etkili bir yaklaşım olduğu belirlendi.
