The relation between serum, seminal plasma, spermatic vein inhibin B levels and improvement of fertility after varicocelectomy in infertile men with varicocele

Abstract
Introduction: The relationship between serum, seminal plasma and spermatic vein İnhibin-B levels and improvement of fertility after varicocelectomy in infertile men with varicocele. The aim of this study is to evaluate the effect of serum, seminal plasma and ejaculate inhibin-B levels on success of fertility after varicocelectomy in infertile men with varicocele. Materials and Methods: The study included 21 infertile men with varicocele and 8 fertile men. Firstly semen analysis was done to all patients and only the patients with oligoasthenoteratozoospermia were programmed for varicocelectomy operation. Inhibin-B levels were analyzed in seminal plasma, peripheric vein blood and spermatic vein blood of the study group. In control group seminal plasma inhibin-B were not studied and only peripheric vein and spermatic vein inhibin-B were studied. The patients who were undergone varicocelectomy operation were called to evaluate in terms of semen analysis and status of their fertility at postoperative 7th month. The correlation between fertility status of patients at postoperative 7th month and their peripheric vein blood spermatic vein blood and seminal plasma inhibin-B levels were tested statistically. Results: The mean inhibin-B levels of peripheric vein blood, spermatic vein blood and seminal plasma in study group were 149.10±132.99 pg/ml (23.12-520.07 pg/ml), 288.17±150.94 pg/ml (51.02-529.29 pg/ml) and 207.63±115.13 pg/ml (71.82-481.31 pg/ml) respectively. The mean inhibin-B levels of peripheric vein, spermatic vein in fertile control group were 157.28±154.46 pg/ml (15.21-511.28) and 313.48±118.52 pg/ml (156.72-498.12) respectively. At the 7th month of varicocelectomy, significant increase of total motil sperm were appeared in 71.4% (15/21) of patients and pregnancy was occurred in 9.5% (2/21) of patients’ wifes. The patients whose wifes were pregnant had the least inhibin-B levels in all three samples. Conclusion: Because the patients who had the least inhibin-B levels were fathered after varicocelectomy operation in our study, we concluded that inhibin-B levels of serum and seminal plasma may be used as a marker to predict success of varicocelectomy operation and we suggest that this issue can be studied in large series to define cutt-off values of inhibin B levels.

Bu çalışmanın amacı, infertil varikoselli erkeklerde varikoselektomiden sonra fertilite düzelmesinin serum, seminal plazma ve spermatik ven inhibin B düzeyleri ile ilişkisini araştırmaktır. Çalışmaya toplam 21 varikoselli infertil hasta ve 8 fertil erkek alındı. Varikoselektomi yapılacak hastalarda önce semen analizi yapıldı ve yalnızca semen ölçütlerinde bozukluk olan hastalar ameliyat programına alındı. Çalışma grubundaki hastalarda seminal plazma, periferik ven ve ameliyat sırasında ise spermatik ven kan örnekleri toplanarak inhibin B düzeylerine bakıldı. Kontrol grubunda seminal plazma inhibin B düzeyi bakılmadı, yalnızca izinleri alındıktan sonra periferik ven ve spermatik venden elde edilen serum inhibin B düzeylerine bakıldı. Varikoselektomi yapılan hastalar 7 ay sonra kontrole çağrıldı. Semen analizi ve fertilite durumları açısından yeniden değerlendirildi. Hastaların 7. aydaki fertilite durumları ile başlangıçta bakılan periferik ven, spermatik ven kanı ve seminal plazma inhibin B düzeyleri arasındaki korelasyon istatistiksel olarak test edildi. Çalışma grubunda periferik ven kanı, spermatik ven kanı ve seminal plazma ortalama inhibin B düzeyleri sırasıyla 149.10±132.99 pg/ml (23.12-520.07 pg/ml), 288.17±150.94 pg/ml (51.02-529.29 pg/ml) ve 207.63±115.13 pg/ml (71.82-481.31) olarak saptandı. Kontrol grubunda periferik ven kanı ortalama İnhibin B düzeyi 157.28±154.46 pg/ml (15.21-511.28 pg/ml) iken, spermatik ven kanı ortalama İnhibin B düzeyi 313.48±118.52 pg/ml (156.72-498.12 pg/ml) idi. Varikoselektomi yapılan hastaların ameliyat sonrası 7. aydaki kontrollerinde %71.4’ünde (15/21) anlamlı total motil sperm artışı oldu, %9.5’inin (2/21) eşinde ise gebelik oluştuğu gözlendi. Gebelik oluşan kadınların eşlerinin inhibin B düzeyleri hem çalışma grubunun hem de kontrol grubunun en düşük inhibin B düzeyleri idi. Sonuç olarak eşinde gebelik saptanan hastaların inhibin B düzeylerinin her üç örnekte de en düşük olması inhibin B’nin varikoselektomi başarısını öngörmede bir belirteç olabileceğine işaret etmektedir. Ancak cutt-off değerleri belirlemek için daha fazla hasta içeren geniş çalışmalara gereksinim olduğunu düşünmekteyiz.
