Subepithelial hematoma of the renal pelvis (Antopol-Goldman lesion): A case report

Abstract
Antopol-Goldman lesion or subepithelial pelvic hematoma of the kidney is a benign lesion and a rare cause of hematuria. Its clinical and radiologic manifestations resemble upper urinary tract neoplasms. A 36-year old woman presented with complaints of macroscopic hematuria and left-sided lumbar pain. A computed tomography scan showed a filling defect of the left renal pelvis. Cystoscopic findings of the bladder were normal. Retrograde pyelography confirmed the presence of the filling defect. Examination of the renal pelvis with flexible ureterorenoscopy failed because of ureteral kinking. A left ureteronephrectomy was performed due to the persistence of hematuria. Histopathologic diagnosis was made as subepithelial hematoma (Antopol-Goldman lesion).
Özet
Antopol-Goldman lezyonu veya böbreğin subepitelyal pelvik hematomu, renal pelvisin iyi huylu lezyonudur ve hematürinin nadir bir nedenidir. Klinik ve radyolojik olarak üst üriner sistem neoplazilerini taklit eder. Otuz altı yaşında kadın hasta makroskobik hematüri ve sol lomber ağrı yakınmasıyla başvurdu. Bilgisayarlı tomografide sol renal pelviste dolma defekti ile uyumlu lezyon vardı. Sistoskopide mesane normal olarak değerlendirilirken, retrograd piyelografide dolma defekti doğrulandı. Fleksibl üreterorenoskop ile renal pelvis değerlendirilmeye çalışıldı; ancak, üreterdeki kıvrımlar nedeniyle başarılı olunamadı. Hematürinin devam etmesi üzerine hastaya sol üreteronefrektomi yapıldı. Histopatolojik incelemede tanı subepitelyal hematom (Antopol-Goldman lezyonu) olarak bildirildi.
