Isolated relapse of a lymphoblastic leukemia T cell precursor in the epididymis

Abstract

A 41-year-old man who had otherwise asymptomatic right scrotal swelling presented to our urology clinic. He had been diagnosed with T cell precursor lymphoblastic leukemia/lymphoma 2 years previous. On ex- amination, his right epididymis was enlarged. A regular, homogeneous, slightly hypoechoic solid mass was observed at the right caput epididymis. This mass measured approximately 7x11x12 mm. Leukemia and lymphoma appear in a variety of locations throughout the body, but a relapse isolated to the epididymis is rare. An epididymectomy was performed, thus removing the patients right-sided sperm duct system. Espe- cially in the younger age group is encountered, the epididymal masses can form fertility problems.

Özet

Kırk bir yaşında erkek hasta sağ tarafta ağrısız skrotal şişlik nedeni ile Üroloji Kliniğine başvurdu. Hastaya 2 yıl önce kemik iliği biyopsisi yapılarak T-hücreli prekürsör lenfoblastik lösemi/lenfoma tanısı kondu. Yapılan fizik muayenede sağ epididimin büyümüş olduğu saptandı. Skrotal ultrasonografide sağ epididimde 7x11x12 mm boyutlarında düzgün konturlu homojen- hafif hipoekoik solid kitle tespit edildi. Lösemi ve lenfoma vücudun değişik bölgelerinde kendini gösterirken, epididimde saptanması oldukça nadirdir. Epidi- dimektomi uyguladığımız bu hastada sağ tarafta sperm kanallarında hasar oluşturuldu. Özellikle genç yaş grubunda karşılaşıldığında, epididimal kitleler fertilite problemi oluşturabilmektedir.
