Results of early open reduction in childrens femoral neck fractures

Abstract
Early stage öpen reduction results of çöllüm femoris fractures ofchildren. We reviewed 9 children's fractures of the femoral neck treated in the department of orthopaedic Surgery and Travmatology of the Dicle Üniversty Medical School during 1991-1996. The main age at the time offracture 9(5-13) and the mean length offollow-up afterfracture was22 month (13-36). The patients were operated in thefırst24 hoursfollowing trauma. Delbet-Colonna's clasifıcation was used to clasify the typ offracture; fourtype2, threetypeS, twotype4.AccordingtoRatliffsclassification,fınalresultsofsixcasesevaluatedgood, onefair, tvopoor. We observed öne avascüler necrosis, öne prematüre physeal closure and coxa vara releated to previos complication. in child deplaced çöllüm femoris fractures, we are of the opinion that intracapsulary hematome drainage. reduction and internalfıxation will reduce the complication rate.
Özet
Dicle üniversitesi Tıp Fakültesi Ortopedi ve Travmatoloji Kliniğinde 1991-1996 yıllan arasında yaşları ortalama 9 (5-13) olan 9 kollum femoris kırıklı hasta travmadan sonra ilk 24 saat içerisinde operasyona alındı. Kırıklar Delbet-Collona sınıflamasına göre guruplandırıldığında, 4'ü Tipli, 3'ü Tiplll, 2'si ise Tip IV olarak değerlendirildi. İntrakapsüler kınk hematomu boşaltıldıktan sonra açık redüksiyon uygulandı. Hastalar ortalama 22 (13-36) ay takip edildiler. Ratlif kriterlerine göre 2 olgu kötü, 1 olgu orta, 6 olgu ise iyi olarak değerlendirildi. Olgularımızdan 1 'inde avasküler nekroz, 1 'inde imlant yetmezliği ve koksa vara, 1 'inde ise erken epifız kapanması gelişti.Çocuk deplase kollum femoris kırıklarında erken dönemde İntrakapsüler hematomun boşaltılması, açık rediiksüyon ve internalfiksasyonun komplikasyon oranını düşüreceği kanaatindeyiz.
