Early diagnosis of colorectal anastomotic leakages by detection of bacterial genome

Abstract

BACKGROUND Microbial infections and translocation ol intestinal bacteria are thought to contribute to multiple system organ failure, but bacterial cultures are often negative in patients with this complication. The purpose of this study was to determine the sensitivity of PÇR for detecting microbial DNA in the blood of animals after conducting an experimental model ol anastomotic leakage. METHODS Fourty rats were divided into three groups as follows: Control Group; simple laparotomy group, Anastomosis Group; colon resection and anastomosis group, and Leakage Group; group with colon resection and an anastomosis leaving a 5 mm opening. Blood was drawn form rats before the procedure, and postoperative 3rd and 6th days. DNAs were extracted from these samples and PGR techniques were used to amplify genes of Escherichia coli. Statistical analysis for the percentage of rats with microbial DNA in the blood for all groups was done by Fisher's exact chi- square test. The difference among groups was considered significant if the P value was less than 0.05. RESULTS Most of the detected Escherichia coli genes were from the Leakage Group, and the detection rate was significant compared to other groups. CONCLUSION We suggest that PGR could be a useful adjunct tool for immediate diagnosis of anastomotic leakages.

AMAÇ Mikrobiyal infeksiyonların ve intestinal bakteriye translokasyonun çoklu organ yetmezliğine yol açan en önemli etkenler olduğu düşünülmektedir. Ancak bakteri kültürleri bu komplikasyonlarm bulunduğu hastalarda çoğunlukla negatif olarak sonuçlanmaktadır. Çalışmamızda deneysel bir anastomoz kaçağı modeli oluşturup, denek kanında etken patojenin (Escherichia coli) DNA'smı PCR ile tespit ederek, bu işlemin anastomoz kaçaklarının önceden tanınmasını sağlayabilecek bir yöntem olup olamayacağını araştırmak istedik. GEREÇ VE YÖNTEM Bu amaçla 40 sıçan, kontrol, anastomoz ve kaçak grubu olarak üç gruba ayrıldı. Kontrol grubuna sadece laparotomi, anastomoz grubuna kolon rezeksiyonu ve anastomoz yapıldı. Kaçak grubunda ise kolon rezeke edildi, ancak anastomoz yapılırken lümenin bir kısmı açık bırakıldı. İşlem öncesinde, işlemin üçüncü ve altıncı günlerinde deneklerden kan örnekleri alındı. Bu örneklerden DNA'lar elde edildi ve Escherichia coli'ye uygun primerlerle reaksiyona tabi tutularak bu patojenin varlığı araştırıldı. İstatistiksel analiz, Fisher'm kesin ki-kare testfyle her bir grupta kanlarında mikrobiyal DNA saptanan deneklerin karşılaştırılmasıyla yapıldı. Gruplar arasındaki fark P değeri 0.05'ten küçük olduğu durumlarda anlamlı olarak kabul edildi. BULGULAR Kaçak grubundan üçüncü ve altıncı günlerde alman kan örneklerinde diğer gruplara göre anlamlı derecede fazla oranda Escherichia coli DNAsına rastlandı. SONUÇ PCR'm bakteriyemiyi kolaylıkla saptayabildiği, bu özelliğiyle de anastomoz kaçaklarının daha önce farkedilmesini sağlayacak bir yöntem olabileceği sonucuna varıldı.
