Synchronous squamous cell lung carcinoma and rectum adenocarcinoma: Case report

Abstract
A 67-year old man patient with dispnea, weakness at the right arm and rectal bleeding, was admitted to our hospital with a nodular large mass in the right upper lung field on chest X-ray. Colonoscopy revealed an ulcerovegetan mass in the rectosigmoid region. Colonoscopic biopsy was performed and the histopathological diagnosis of a specimen was evaluated as adenocarcinoma. Chest computed tomography revealed an 8 cm mass lesion with heterogenous density in the right upper lung field. Transbronchial biopsy of the specimen revealed squamous cell carcinoma. The patient was diagnosed as synchronous squamous cell lung cancer and rectum adenocarcinoma, who was treated with only supportive therapy due to his poor performance status. Complete blood count showed excessive leukocytosis that was evaluated due to paraneoplastic syndrome. He died shortly after diagnosis. Synchronous lung and rectum cancers together with paraneoplastic leukocytosis was rarely reported in the literature.
Özet
Nefes darlığı, sağ kolda güçsüzlük ve rektal kanama öyküsü olan 67 yaşında erkek hasta akciğer grafisinde sağ akciğer üst lobda kitle ile hastanemize kabul edildi. Yapılan tetkiklerde rektosigmoid ülserovegetan kitle ve sağ akciğer üst lobda 8 cm çaplı kitle tespit edildi. Transbronşial iğne biyopsisinde skuamöz hücreli kanser saptandı. Kolonoskopik biyopsi sonucu adenokanser olarak rapor edildi. Senkron kolon adenokarsinomu ve akciğer skuamöz hücreli kanseri tanısı kondu. ECOG performans statusu 3 olması nedeni ile sadece destek tedavisi verildi. Hastada enfeksiyon bulgusu yoktu. Aşırı lökositoz paraneoplastik sendrom olarak değerlendirildi. Hasta, destek tedavisi altında iken öldü.Senkron akciğer kanseri ve rektum kanserinin paraneoplastik lökositoz ile birlikteliği literatürde nadiren bildirilmiştir
