Abstract

Aim: The term fetus papyraceus is used to describe a flattened, mummified fetus associated with a viable twin or multiple gestation which is due to intrauterine fetal demise in the second trimester. We will review the pregnancy management of this rare entity and the related literature which we believe we will encounter, more in the future due to increase in the incidence of multiple gestation. Case report: In this case report we will present a dichorionic-diamniotic twin gestation in which fetal demise of one of the twins occurred in the 15th gestational week and detected as a fetus papyraceus attached to the placenta at birth. Conclusion: We believe determination of chorionicity and gestational age in the early pregnancy plays the key role in the management of twin pregnancies regarding fetus papyraceus and similar presentations.

Özet

Amaç: Çoğul gebeliklerde fetuslardan birinin 2. trimesterde ölümü ve ölen fetusun yumuşak dokularının büyük kısmının kaybı, maserasyonu ve yassılaşması fetus papiraseus olarak tanımlanmaktadır. Nadir görülen ancak artan çoğul gebelik insidansı nedeniyle klinikte daha sık karşılaşacağımız bu olgularda gebelik yönetimi literatür eşliğinde tartışılmıştır. Olgu sunumu: Bu yazıda 15. gebelik haftasında ikiz eşinin intrauterin ölümünün gerçekleştiği dikoryonik-diamniotik gebelikte, doğumda plasentaya yapışık halde bulunan fetus papiraseus olgusu sunulmaktadır. Sonuç: Çoğul gebeliklerde erken dönem ultrasonografi ile koryonisite ve gebelik yaşının belirlenmesinin, fetus papiraseus ve benzeri gelişebilecek komplikasyonların yönetiminde anahtar rol oynadığını düşünmekteyiz.
