Prognostik inflamatuvar ve nütrisyonel indeks'in çocuklarda postoperatif morbiditeyle ilişkisi

Abstract

This study was undertaken to determine the safety of prognostic inflammatory and nutritional index (PINI) as an indicator of metabolic response to surgical stress, and whether there is a relationship between PINI and postoperative morbidity in children. Thirty-two patients who were performed appendectomy were enrolled in the study. Of the patients 17 had acute appendicitis (Group 1) and 15 had gangrenous or perforated appendicitis (Group 2). Serum albumin (AL), prealbumin (PA), c-reactive protein (CRP) ve alpha-1 acid glicoprotein (AGP) values were measured preoperatively (-1), on postoperative day 1 (+1), and postoperative day 3 (+3) in all patients. Mean PINI-1, PINI+1, and PINI+3 values were calculated by using these parameters. In grup 1, mean PINI-1, PINI+1, and PINI+3 values were fonud 5.6±3.9, 15.6±7.8, and 5.8±3.6, respectively (p<0.0001). In group 2, the same values were 25.2±16.4, 47.2±28.1, and 43.9±24.2, respectively. The difference between PINI-1 and PINI+1 was found statistically significant (p<0.0001) while there was no significant difference between PINI+1 and PINI+3 (p>0.02) in group 2. Mean PINI values were found higher in group 2 compared with group 1, and the difference between two groups was statistically significant (p<0.001). These results suggest that PINI is a safe and valuable index for determination of metabolic response to surgical stress in children, and postoperative follow-up of surgical patients.

Özet

Çalışmamızda, çocuklarda cerrahi stres sonucu ortaya çıkan metabolik yanıtdaki değişiklikleri göstermede, prognostik inflamatuvar ve nütrisyonel indeks (PINI)'in güvenilirliğini ve postoperatif morbiditeleri ne oranda ortaya koyduğu araştırılmıştır. Akut karın nedeni ile başvuran ve apandektomi yapılan 32 hastada çalışıldı. Akut apandisit (Grup 1, n=17) ve gangrenöz veya perfore apandisit saptanan (Grup 2, n=15) hastalardan preoperatif (-1), postoperatif 1. gün (+1) ve 3. gün (+3) kan alınarak serum albumin (AL), prealbumin (PA), c-reaktif protein (CRP) ve alfa-1 asid glikoprotein (AGP) değerleri saptanarak, ortalama PINI-1, PINI+1 ve PINI+3 değerleri hesaplandı. Grup 1'de ortalama PINI-1, PINI+1 ve PINI+3 değerleri sırasıyla 5.6±3.9, 15.6± 7.8 ve 5.8±3.6 bulundu (p 0.02) olduğu bulundu. Her iki grubun ortalama PINI değerleri karşılaştırıldığında, Grup 2'deki değerlerin Grup 1 değerlerine göre büyük ve farkların anlamlı (p<0.001) olduğu bulunmuştur. Bu veriler so-nucunda, PINl'ın çocuklarda, cerrahi strese metabolik yanıtın ortaya konmasında ve hastaların postoperatif takibinde değerli ve güvenilir bir indeks olduğunu göstermektedir.
