Abstract

Aim: In this case report, it is aimed to discuss the ‘HaNDL Syndrome’ with the relevant literature. Case Report: A 20 year-old woman was admitted to the emergency department with left-sided, severe, throbbing headache accompanied with nausea, vomitting, sonophophia and phonophophia. Approximately 30 minutes after the pain emerged, she had suffered numbness and mild paresis on the right side of the body including the right side of her face accompanied with difficulty in speech which improved in approximately 20-25 minutes. She reported that 2 weeks prior these symptoms, she had suffered severe nausea and diarrhea that had lasted for 3 days. A diagnosis of viral gastroenteritis had been made and supportive therapy had been advised by a doctor. In the clinic, spinal tap was performed which revealed lymphocytic pleocytosis and mild elevation of protein level (49 mg/dL). Brain computerized tomography and the contast MR were normal. After the exclusion of other conditions which may cause CSF pleocytosis and severe headache; a diagnosis of HaNDL Syndrome was made. Conclusion: Recognition of this bening syndrome, is important as performing a diagnostic angiography may cause severe complications.

Özet

Amaç: Bu vaka sunumunda, HaNDL Sendromunun (Baş ağrısı, nörolojik defesit, BOS’ ta lenfositoz) literatür eşliğinde tartışılması amaçlanmıştır. Olgu Sunumu: 20 yaşında bayan hasta, acil servise başının sol tarafında olan, bulantı-kusma, sonofobi ve fonofobinin eşlik ettiği şiddetli, zonklayıcı karakterde başağrısı nedeniyle başvurdu. Ağrının başlamasından yaklaşık olarak 30 dakika sonra vücudunun sağ tarafında, bacaklarından başlayıp kola ve yüze yayılan uyuşma ve hafif güçsüzlük ile konuşmada bozulma olduğu, bu şikayetlerinin 20-25 dakikada düzeldiği öğrenildi. Hastanın bu olaydan ortalama 2 hafta önce, yaklaşık olarak 3 gün kadar devam eden şiddetli bulantı ve diyarenin olduğu viral gastroenterit geçirdiği, hekime başvurduğu ve destek tedavi dışında bir tedavi almadığı öğrenildi. Klinikte yapılan lomber ponksiyonda lenfositik pleositoz ve protein düzeyinde yükseklik (49 mg/dL) saptandı. Hastanın acil serviste çekilmiş olan BBT’si ve kontrastlı MR’ı normaldi. BOS pleositozuna ve şiddetli başağrısına neden olabilecek diğer nedenlerin dışlanması sonucunda hastaya HaNDL Sendromu tanısı kondu. Sonuç: Benign bir sendrom olan HaNDL Sendromunun tanınması, tanı amaçlı yapılacak olan anjiografi gibi tetkiklerin getirebileceği ciddi riskler nedeniyle önem taşımaktadır.
