Comparison of intrathecal hyperbaric bupivacaine and hyperbaric bupivacaine+sufetanil combination in lower extremity surgery

Abstract

We aimed to evaluate the effects of intrathecal sufentanil added to hyperbaric bupivacaine for lower extremity surgery. After obtaining approval from the ethics committee, fifty patients (ASA l-ll) were included in this study and were allocated randomly to two groups of 25 each. 10 mg hyperbaric bupivacaine+ 10 $mu$g sufentanil was injected to Group S and the same dose of hyperbaric bupivacaine given to Group K with 2 mL %0.9 NaCI, four mL solution was used in both groups for İT anesthesia. Heart rate (HR), systolic blood pressure (SBP), diastolic blood pressure (DBF], mean blood pressure (MBP), peripheral oxygen saturation (SpO2), sensory and motor block were recorded every 5 minutes during first 30 min, every 10 min for the following 30 min, and every 15 min intervals In the operation period. Intraoperative/postoperative side effects and postoperative analgesic requirements were recorded. HR, SBP, DBF and MBP were decreased significantly in both groups when compared with control values and no significant difference were noted between the groups. The onset time of sensory block was shorter in group S and sensory block was higher in Group S than Group K. The duration of analgesia was longer in Group S than Group K (p<0.05}. In conclusion: intrathecal 10 $mu$g sufentanil added to hyperbaric bupivacaine for lower extremity surgery increased the duration of analgesia in the early postoperative period, l0 $mu$g sufentanil plus hyperbaric bupivacaine provided to improve both the quality and duration of analgesia in this procedures. Because this combination had no adverse effect except pruritus, we concluded that it can be used safety in lower extremity surgery.

Özet

Bu çalışmada alt ekstremite cerrahisinde intratekal (İT) hiperbarik bupivakaine eklenen sufentanilin etkilerinin araştırılması amaçlandı. Etik komite onayı alındıktan sonra çalışmaya ASA l-ll sınıfına uyan 50 olgu alındı ve randomize olarak 25'şer kişilik iki gruba ayrıldı. Grup S'e l0 mg (2 mL/10 mg) hiperbarik bupivakain+ 10 $mu$g (2 ml/10 $mu$g) sufentanil ve Grup K'ya aynı miktardaki hiperbarik bupivakain 2 mL %0.9 NaCI ile verildi. Her iki grupta da 4 mL volüm İT anestezi için uygulandı. Kalp atım hızı JKAH), sistolik arter basınçları (SAB), diastoiik arter basınçları (DAB), ortalama arter basınçları (OAB) ve periferik oksijen satürasyonları (SpO2), sensoryal ve motor blok ilk 30 dakikada her 5 dakikada bir, takip eden 30 dakikada 10 dakikada bir, sonraki operasyon periyotlarında ise her 15 dakikada bir kaydedildi, intraoperatif ve postoperatif yan etkiler ve postoperatif analjezik gereksinimleri belirlendi. Kontrol değerine göre her iki grupta KAH, SAB, DAB, OAB anlamlı derecede düştü, fakat gruplar arasında bu parametreler açısından fark yoktu. Grup S'de sensoryal blok başlama zamanı daha hızlıydı. Analjezi süresi Grup S'de Grup K'ya göre daha uzun bulundu (p<0.05). Sonuç olarak; alt ekstremite cerrahisi için İT hiperbarik bupivakaine lO $mu$g sufentanil ilave edilmesi bu işlemlerde hem analjezi kalitesini artırmış hem de erken postoperatif analjezik gereksinim zamanını uzatmıştır. Bu kombinasyonla kaşıntı dışında anlamlı bir yan etki görülmediğinden alt ekstremite cerrahisinde güvenle kullanılabileceği kanaatine varılmıştır.
