Pulmonary hydatidosis: Surgical treatment and follow up of 728 cases

Abstract
For explorating results of hydatid cyst operations, between 1970-1999, 728 patients were reviewed. Male/female ratio: 2.14; mean age 29.25 ± 5.68 years (5 mo-71 yr) and the most frequent symptom was fever. The incidence of concomitant liver and lung hydatid cysts were 6.73% (49 cases) and performed right thoracophrenotomy (n= 26); median sternotomy and right phrenotomy (n= 5) for them. For bilateral lung hydatid cysts, 8 of 31 patients were underwent one-stage bilateral thoracotomy, 11 of them sequential staged bilateral thoracotomy and 12 of them median sternotomy. The most frequent operation method was cystotomy and capitonnage (82.5%). Cystotomy without capitonnage in 51 patients (10.4%), pulmoner resection with 7%, and wedge resection in 9% were performed. We performed cystotomy without capitonnage in 76 (11.2%) patients, pulmonary resection in 51 (7%) and wedge resection in 9 (1.2%). Atelectasis and wound infection were most frequent postoperative complications. The rate of morbidity 14.4%, mortality 1.5%, recurrence 2.5% and mean hospital stay were 13.4 ± 7.2 (9-40) days.

Hidatik kist operasyonlarının sonuçlarını incelemek için, 1970-1999 yılları arasında opere edilen 728 hasta incelendi. Erkek/kadın oranı: 2.14; yaş ortalaması 29.25 ± 5.68 yıl (5 ay-71 yıl); en sık semptom ateşti. Akciğer ve karaciğer kisti birlikteliği %6.73 (49 olgu) idi ve bunlarda sağ torakofrenotomi (n= 26); median sternotomi + sağ frenotomi (n= 5) yapıldı. Bilateral akciğer kisti olan 31 hastanın 8'inde tek evreli bilateral torakotomi; 11'inde iki evreli torakotomi 12'sinde median sternotomi yapıldı. En sık operasyon yöntemi, kistotomi ve kapitonajdı (%82.5). Kapitonajsız kistotomi 76 (%11.2); akciğer rezeksiyonu 51 (%7) olguda, wedge rezeksiyon ise 9 (%1.2) hastada yapıldı. En sık görülen postoperatif komplikasyonlar atelektazi ve yara infeksiyonu olup, morbidite %14.4, mortalite %1.5 ve rekürrens %2.5 idi. Hastanede kalış süresi 13.4 ± 7.2 (9-40) gündü.
