General practitioners clinical approach to asthma in Kocaeli

Abstract
The questinonnarie from consisting of 31 questions was given to 126 general practitioners (GP) working in state hospitals and health centers in Kocaeli with the aim of evaluating the knowledge and attitude of physicians against asthma. The physicians were divided into two groups as: Those who have participated in postgraduate courses previously (group 1) and those who have not (group 2). In group 1 among 39 GP, 29 (74.4%) and in group 2 among 87 GP, 66 (75.9%) stated that they had the diagnosis of asthma in their patients and in group 1, 24 (61.5%) and group 2, 52 (59.8%) GP stated that they started asthma treat-ment to their patients. In group 1, 27 (69.2%) and group 2, 59 (67.8%) crossed the true statement for definition of asth-ma, on the other hand 18 (46.2%) GP in group 1 and 40 (46.0%) GP in group 2, crossed the true statement for asth-ma pathogenesis. In definning acute attack treatment of asthma the GP who selected inhaled beta-2 agonist as the first step treatment in asthma were 18 (46.2%) in group 1 and 38 (43.7%) in group 2. Although there was a great similarity m clinical approach of two groups to asthmatic patients; there was a statisticaly difference between two groups in using antiinflammatory agents in asthma maintenance therapy (p= 0.020)These results have indicated us that the general practitioners had significant lack of knowledge about diagnosis, pathogenesis and treatment of asthma and postgraduate courses must be carried in their future education.

Kocaeli ilindeki çeşitli kurum ve kuruluşlarda görev yapan pratisyen hekimlerin astma konusundaki bilgi, tutum ve davranışlarını araştırmak amacıyla 126 pratisyen hekime 31 sorudan oluşan bir anket uygulanmıştır. Hekimler son bir yıl içinde astma ile ilgili herhangi bir eğitim programına katılanlar (grup 1) ve katılmayanlar (grup 2) olmak üzere iki gruba ayrılmıştır. Birinci gruptaki 39 hekimin 29 (%74.4)'u, 2. gruptaki 87 hekimin 66 (%75.9)'sı astma tanısı koyduğunu belirtirken, 1. gruptan 24 (%61.5), 2. gruptan da 52 (%59.8) hekim astmalı hastaları tedavi ettiklerini bildirmişlerdir. Birinci gruptan 27 (%69.2), 2. gruptan 59 (%67.8) hekim astma tanımını doğru işaretlerken, astma patogenezini doğru işaretleyenlerin sayısı 1. grupta 18 (%46.2), 2. grupta ise 40 (%46.0) bulunmuştur. Akut atak tedavisinde ilk tercih olarak inhale beta-2 agonist işaretleyenlerin sayısı ise 1. grupta 18 (%46.2), 2. grupta ise 38 (%43.7) bulunmuştur. Her iki grubun astmalı hastaya yaklaşımlarında büyük oranda benzerlik bulunmuş olup, yalnızca astmanın idame tedavisinde antiinflamatuvar ilaç kullanımı konusunda iki grup arasında istatistiksel olarak anlamlı fark bulunmuştur (p= 0.020). Sonuçlarımız pratisyen hekimlerin astma tanımı, patogenezi ve astmalı hastaya yaklaşım konularında önemli eksikliklerinin olduğunu ve mezuniyet sonrası eğitim programlarına daha fazla önem verilmesi gerektiğini ortaya koymuştur.
