Subclavian vein catheterization and complications

Abstract
Central venous catheters are one of the most significant advances in the field of intravenous therapy. We revived our experience on subclavian vein catheterization to assess safety and complications of procedure. Four hundred seventy three patients undergoing a subclavian vein catheterization in Gülhane Military Medical Academy Thoracic Surgery Department, between January 2000 and November 2001. Catheter was inserted via subclavian vein by Seldinger technique with a three lumens central venous catheter in all cases. Seven patients (1.47%) experienced immediate procedural complications; minor pneumothorax 1, hydrothorax 1 and catheter malposition in five patients during follow-up. No major complication and mortality occurred. Subclavian vein catheterization is still a safe procedure in experienced hands but it is necessary to take a great care not to overlook its immediate and delayed mechanical complications.

Santral venöz kateterizasyon, intravenöz tedavide etkili bir yöntemdir. Biz bu çalışmada subklavian kateter uygulamalarımızı ve gelişen komplikasyonları inceledik. Ocak 2000-Kasım 2001 tarihleri arasında Gülhane Askeri Tıp Akademisi Göğüs Cerrahisi Anabilim Dalı'nda 473 olguya subklavian ven kateterizasyon uygulandı. Tüm olgularda, santral yol olarak subklavian vene, 3 lümenli santral kateter, "Seldinger" tekniği ile uygulandı. Toplam 7 olguda çeşitli komplikasyonlar gözlendi. Bir olguda pnömotoraks, 1 olguda hidrotoraks ve 5 olguda malpozisyon tespit edildi. Majör komplikasyon ve mortalite gözlenmedi. Yoğun bakım takibi gereken ve uzun süreli takibi zorunlu olan hastalarda subklavian venöz santral kateterizasyon tecrübeli ve dikkatli ellerde güvenle uygulanabilecek küçük cerrahi girişimdir ve girişim sonrası kateter lokalizasyonu ve oluşabilecek komplikasyonlar kontrol edilmelidir.
