The effect of utilization of wide metalic substernal bar in pectus excavatum

Abstract
Retrospective analysis of 24 cases operated on due to the diagnosis of pectus excavatum deformity between March 1996-january 2000 has been presented. Following application of Ravitch procedure for correction; Kirshner wires in 14 cases and wide metallic sternal bars were used in 10 cases in order to achieve sternal elevation and strenghten the fixation. Mean follow-up period of the cases was 9 ± 2 (6-11) months. In the postoperative period there was morbidity in two patients (8%) but no mortality was observed. Control check up examination of the cases has shown that, correction of the deformation was more effective in the cases operated on by use of sternal bars. We have concluded that; wide sternal bars has the advantage of stronger sternal fixation capability and more effective sternal elevation properties when compared to Kirshner wires in terms of pectus correction operations, especially the operations performed throughout early post-childhood period.

Kliniğimizde Mart 1996-Ocak 2000 tarihleri arasında pektus ekskavatum nedeniyle opere edilen 24 olgu retrospektif olarak incelendi. Ravitch yöntemi ile korreksiyon uygulamasını takiben sternum elevasyonunu sağlamak ve fiksasyonu kuvvetlendirmek için olguların 14'üne Kirschner teli, 10 olguya ise geniş metalik sternal bar kullanıldı. Olgular ortalama 9 ± 2 ay (6-11 ay) takip edildi. Postoperatif dönemde morbidite 2 hastayla %8 bulundu, mortalite görülmedi. Yapılan takipler sonucunda sternal bar kullanılan olgularda deformasyondaki düzelmenin daha efektif olduğu gözlemlendi. Özellikle çocukluk çağı sonrasında yapılan pektus operasyonlarında geniş sternal barın, Kirshner teline göre daha kuvvetli bir sternal fiksasyonla daha efektif bir sternum elevasyonu sağladığı kanısına varıldı.
