Evaluation of the efficacy and tolerance of cisplatin / vinorelbine combination in nonsmall cell lung cancer

Abstract

The purpose of this study is to evaluate efficacy, toxicity of the combination chemotherapy regimen with cisplatin/vinorelbine (CDDP/VNR). Thirty one consecutive patients (27 men, 4 women) were enrolled. 20 patients were stage 4, 11 were stage 3B. Patients were treated with CDDP 80 mg/m2 on day 1 and VNR 25 mg/m2 on day 1 and 8. The cycle was repeated every 21 days. One hundred five courses were completed. Grade 2 and more hematologic toxicity occured in 30/105 courses. Objective response rate was 58% with complete remission in 3.2% and partial response in 54% patients. Three patients (9.7%) had stable diseases and 10 patients (32%) had progression. The median response duration was 6 months (2-9 months) in the evaluated 13 patients. The median overall survival was 12 months. These findings suggest that the combination of CDDP and VNR appears to be suitable for the first line treatment of locally advanced and metastatic NSCLC.

Özet

Bu çalışmada amaç, ileri evre küçük hücreli dışı akciğer kanserlerinde (KHDAK); sisplatin/vinorelbin (CDDP/VNR) kombine kemoterapi rejiminin etkinliğini, güvenilirliğini ve toksisitesini değerlendirmektir. Çalışmaya 4'ü kadın, 27'si erkek, toplam 31 hasta alınmıştır. Hastaların 20'si evre 4, 11 'i evre 3B'dir. Hastalara 80 mg/m2 CDDP 1. gün ve 25 mg/m2 VNR 1. ve 8. günler olmak üzere 21 günlük sikluslar halinde toplam 6 kür kemoterapi (KT) verilmiştir. Otuzbir hastanın 1 (%3.2)'inde tam remisyon, 17 (%54.8)'sinde parsiyel yanıt, 3 (%9.7)'ünde stabil hastalık, 10 (%32)'unda progresyon gelişirken, median remisyon süresi, yanıtları değerlendirilebilen 13 hastada 6 ay (2-9 ay) olarak saptanmıştır. Median sağkalım süresi 12 aydır. Uygulanan 105 siklusun 30 (%29)'unda grade 2 (WHO) ve üzeri hematolojik toksisite, 2 (%2)'sinde grade 2 nefrotoksisite; 31 hastanın 9 (%29)'unda cilt toksisitesi gelişmiştir. Sonuç olarak, elde edilen yüksek yanıt oranı, düşük toksisite ve oldukça iyi tolere edilmesi nedeniyle CDDP/VNR kombinasyonu ileri evre KHDAK'lar için ilk basamak tedavi seçeneği olarak uygun bulunmuştur.
