The pulmonary toxicity pattern due to amiodarone

Abstract

Amiodarone is an antiarrhythmic agent for the control life threatening arrhytmia, which could not be treated with other antiarrhythmic drugs, and it has a risk for pulmonary toxicity. In this study, the lung and systemic toxicity of amiodarone treatment is investigated and compared with the dose and duration of treatment. The study was enrolled on 18 nonsmoker patients (13 male and 5 female) at mean age 59 ± 6 years and treatment duration of 7.2 ± 6 months. Arterial blood gas analyses, pulmonary function tests, chest X-ray, thorax high resolution computed tomography (HRCT) and 67Ga scintigraphy were used to evaluate the pulmonary toxicity. Eleven (61.1%) patients had clinical symptoms of pulmonary toxicity, 5 patients (28%) arterial blood gas disturbance, 9 patients (50%) decreased lung volumes and 10 pati-ents (55.6%) decreased single-breath diffusing capacity. The HRCT of thorax showed ground-glass appearance, widening of septa, parenchymal infiltration and nodular densities in 14 of (78%) 18 patients. Abnormal gallium uptake was found in 14 patients (78%) at 67Ga scintigraphy. Pulmonary toxicity is established if three of 5 investigated methods areabnormal and 6 patients (33%) met these diagnostic criteria. Liver function test elevation, eosinophilia, thrombositopenia were detected in 11 patients (62%) and thyroid function disturbance was found in 8 patients and this findings were interpreted as systemic toxicity. In conclusion, the duration of treatment with amiodarone was longer (11+7 months) in patients who had pulmonary toxicity findings on CT scanning than patients who had not (4.3 ± 2 months). The patients with an abnormal thyroid function test were treated longer (12.4 ± 1 months) when compared with control subjects (7.2 ± 1 months) (p value 0.02 and 0.04, respectively).

Hayatı tehdit eden ve diğer antiaritmiklerle kontrol altına alınamayan aritmilerde kullanılan amiodaron, akciğer toksisite potansiyeline sahip bir ilaçtır. Bu çalışmada, amiodaron kullanım dozu ve süresi ile akciğer ve sistemik toksisite gelişimi arasındaki ilişki araştırılmıştır. Çalışmaya; sigara içmeyen, ortalama tedavi süresi 7.2 ± 6 ay, 13'ü erkek, yaş ortalamaları 59 ± 6 yıl olan toplam 18 olgu alınmıştır. Akciğer toksisitesinin araştırılması için, arteryel kan gazı (AKG) analizi, solunum fonksiyon testi (SFT), akciğer grafisi, toraks yüksek rezolüsyonlu bilgisayarlı tomografisi (YRBT), galyum (67Ga) sintigrafısi yapılmıştır. Onbir olguda (%61.1) akciğer toksisitesini düşündürecek klinik semptoma rastlanırken, 1 olguda (%5.5) AKG bozukluğu (hipoksi), 9 olguda (%50) akciğer volümlerinde ve 10 olguda (%55.6) difüzyon kapasitesinde azalma şeklinde SFT patolojisi saptanmıştır. Onsekiz olgunun 10 (%55.6)'unda akciğer grafisinde patoloji, 14 (%77.8)'ünde YRBT'de toksisiteyi düşündüren buzlu cam görünümü, septal kalınlaşma, parankimal infiltrasyon, nodüler yapılar görünmüştür. 67Ga tutulumu ise 14 hastanın 4 (%28.6)'ünde gösterilmiştir. Bu 5 inceleme yönteminden en az 3'ünde patoloji saptanması akciğer tutulumu lehine değerlendirilmiş, 8 olguda (%44.4) akciğer toksisitesi düşünülmüştür. Yanısıra, 11 olguda (%61.1); eozinofili, trombositopeni, karaciğer enzimlerinde yükselme ve 8 olguda (%44.4); tiroid fonksiyon testi bozukluğu şeklinde sistemik toksisiteyi düşündüren bulgular saptanmıştır. Amiodaron kullanım süresi; toraks YRBT'de akciğer toksisitesini düşündüren bulgular bulunanlarda (11 ± 7ay) bulunmayanlardan (4.3 ± 2 ay), tiroid fonksiyon testlerinde bozukluk saptananlar da (12.4 ± 1 ay) saptanmayanlardan (7.2 ± 1 ay) uzun bulunmuştur (sırasıyla p= 0.02 ve p= 0.04).
