The sputum results and the affecting factor of them in pulmonary tuberculosis patients

Abstract
Microscopic examination of a stained sputum smear for acid fast bacilli (AFB) and tuberculosis culture are gold standard methods in the diagnosis and treatment monitoring of active pulmonary tuberculosis. In this study we investigated 873 pulmonary tuberculosis patients retrospectively in the last 12 years. AFB was positive in 441 (50.5%) and tuberculosis culture was positive in 614 (70.3%) patients. AFB and culture positivity were not effected from the patients' age, gender, education, hemoptysis and comorbidity (p> 0.05). AFB and culture positivity were higher in the patients who had cough, sputum, and cavity on the chest X-ray (p< 0.05). Bacilli conversion in smears and culture were seen in 1.87 ± 0.89 and 2.84 ±1.2 months respectively. The patient age, gender, symptoms, close contact with tuberculosis patients, comorbidity and roentgenographic manifestations did not effect the time of bacilli conversion. However a positive sme-ar and prior history of tuberculosis were the factors associated with increased number of days for culture conversion.
Özet
Akciğer tüberkülozu (TB)'nun kesin tanısında ve tedavi etkinliğinin takibinde altın standart balgam kültür sonuçlarıdır. Biz bu çalışmamızda son 12 yıl içerisinde dispanserimizde takip ve tedavi edilen 873 akciğer TB olgusunun balgam sonuçlarını retrospektif olarak incelemeyi amaçladık. Bu hastaların 441 (%50.5)'inde balgam yaymasında aside dirençli basil (ARB), 614 (%70.3)'ünde ise kültür sonuçları pozitif idi. Yayma ve kültür pozitifliğini hastanın yaş, cinsiyet, eğitim durumu, hemoptizi şikayeti ve ek hastalık varlığı etkilemiyordu (p> 0.05). Ancak hastada öksürük, balgam çıkarma şikayetleri olan ve radyolojik olarak kavile bulunan hastalarda daha yüksek oranlarda yayma ve kültür pozitifliği saptandı (p< 0.05). Tedavi ile balgam yayması negatifleşmesinin 1.87 ± 0.89 ayda, kültür negatifleşmesinin ise 2.84 ± 1.2 ayda gerçekleştiği saptandı. Hastalarda balgam yayma ve kültür sonuçlarının negatifleşme zamanını yaş, cinsiyet, semptom, TB'Iİ hasta ile temaslı olma, ek hastalık ve radyolojik bulguların etkilemediği, sadece önceden TB geçiren ve yayma pozitif olgularda daha uzun olduğu tespit edildi.
