The effect of patient and doctor on delayed diagnosis in lung cancer

Abstract

There is a delay in diagnosis and treatment of lung cancer. In this study; patient's and doctor's effect to the delay in diagnosis of lung cancer was detected. The relation of these delays, the treatment used before diagnosis with the stage of the disease and the presence of metastasis were searched. Of the patients those were diagnosed lung cancer, the demographic data, symptoms and the information about the first doctor visit were collected. The average age of 199 patients was 61.4 years. The diagnosis was achieved in 12.4 days. After the presence of first symptom, patients had contacted to doctor with a delay about 56.7 days (patient's delay). Between the first contact to doctor and diagnosis, there was 48 days, and from the first symptom to diagnosis, there was 104.7 days. When the ages and gender were stable, there were not any differences for delays in diagnosis according to the tumor type or the presence of metastasis (p> 0.05). After the first contact to doctor, 58.3% of the cases had been directed to diagnostic processes without treatment. In 41.7%, treatment was advised including analgesic/antiinflammatory (55.2%) and antibiotics (53.7%). The period bet¬ween first contact to doctor and diagnosis was 74.6 days in group those had been advised treatment and was 29 days in group those had been directed to diagnostic processes (p= 0.000). The period between first symptom and diagnosis was 122.9 days and 91.6 days, respectively (p= 0.01). The results of this study indicate that; the diagnosis of lung cancer is achieved three and a half months after first symptom; the period between first symptom and diagnosis do not effect the presence of metastasis, performance status and stage of disease; patient's delay and the period between first contact to doctor and diagnosis are not related with stage of disease, the presence of metastasis; diagnosis is delayed in cases those used treatment.

Akciğer kanseri tanı ve tedavi aşamasında gecikme söz konusudur. Bu çalışmada, akciğer kanserinde tanı gecikmesine olguların ve hekimin etkisi gözden geçirilmiştir. Ayrıca, bu gecikmelerin ve tanı öncesi uygulanan tedavinin hastalığın evresi ve metastaz varlığı ile ilişkisi araştırılmıştır. Kliniğimizde akciğer kanseri tanısı alan olguların demografik verileri, semptomları ve hekim başvuruları ile ilgili bilgiler kaydedildi. Çalışmaya alınan 199 olgunun yaş ortalaması 61.4 yıl idi. Olgulara ortalama 12.4 günde tanı konuldu, ilk semptom ortaya çıktıktan sonra olgular ortalama 56.7 günde hekime başvurmuşlardı (olguya ait gecikme). İlk hekim başvurusundan tanı koyulmasına kadar ortalama 48 gün, ilk semptomdan tanı koyulmasına kadar ortalama 104.7 gün geçmişti. Olguların yaşı ve cinsiyetleri sabit tutulduğunda; tümör tipine göre ya da metastaz varlığına göre tanı gecikme süreleri farklı değildi (p> 0.05). İlk hekim başvurusu sonrası olguların %58.3'ü herhangi bir tedavi düzenlenmeden ileri tetkik için yönlendirilmişti. Olguların %41.7'sine en sık analjezik/antiinflamatuvar (%55.2) ve antibiyotik (%53.7) olmak üzere çeşitli tedaviler önerilmişti. Hekime ilk başvurudan tanıya kadar geçen süre; tedavi önerilen grupta 74.6 gün, tedavi düzenlenmeden tetkik için yönlendirilen grupta 29 gündü (p= 0.000). İlk semptomdan tanıya kadar geçen süre ise sırasıyla 122.9 gün ve 91.6 gündü (p= 0.01). Bu çalışmada; akciğer kanserli olgulara ilk semptomdan ortalama 3.5 ay sonra tanı koyulduğu, ilk semptomdan tanıya kadar geçen sürenin metastaz varlığı, performans düzeyi ve hastalığın evresi ile ilişkili olmadığı, hekime ilk başvurudan tanıya kadar geçen süre ve olguya ait gecikmelerin hastalığın evresi, metastaz varlığı ile ilişkili olmadığı, semptomları nedeniyle tedavi düzenlenen olgulara daha geç tanı konulduğu tespit edilmiştir.
