Diagnostic value of pleural adenosine deaminase (pADA), serum adenosine deaminase (sADA) and pleura/serum adenosine deaminase (p/s ADA) ratio in tuberculous and nontuberculous pleural effusion

Abstract

We aimed to determine the cut-off value of pleural adenosie deamiase (pADA) in our country with a high prevalence of tuberculosis and to estimate the diagnostic efficiency of serum adenosine deaminase (sADA) and pleural/serum adenosine deaminase (p/s ADA) ratio for differential diagnosis of tuberculous pleural effusions. We analyzed clinical and analytic data of 398 consecutive patients with pleural effusions from November 2001 to January 2003. The study population including 72 tuberculous pleural effusions, 147 malignant pleural effusions, 45 parapneumonic pleural effusions, 18 other exudates and 116 patients with transudates. The means and standard deviations of pADA, p/s ADA ratio and sADA were evaluated according to their underlying diseases. There were no significant differences in s ADA values between each group. pADA levels and p/s ADA ratio were significantly higher in patients with tuberculous pleural effusions than other patients (ANOVA, p< 0.000). Receivig operating characteristic (ROC) curve identified 35 U/L as the best cut-off value for pADA in tuberculous pleural effusions and yielded a sensitivity and specificity of 84.7 % and 91.1%, respectively. Taking a cut-off Value 1.4 for p/s ADA ratio, the sensitivity and the specificity were found to be 80.6% and 84.4%, respectively İn tuberculous pleural effusions. ROC curve identified 35 U/L as the best cut-off value for pADA in detecting tuberculous pleural effusions, p/s ADA ratio was not superior than pADA alone in distinguishing tuberculous pleural effusions. When p/s ADA ratio and pADA evaluated together we found higher sensitivity than pADA alone in diagnosis of tuberculous pleural effusion.

Tüberküloz (TB) prevalansının yüksek olduğu ülkemizde pleVral adenozin deaminaz (pADA) eşik değerini belirlemeyi ve TB plevral efüzyonun ayırıcı tanısı İçin serum adenozin deaminaz (sADA) ve plevra/serum adenozin deaminaz (p/s ADA) oranının tahısal etkinliğini araştırmayı amaçladık. Kasım 2001-Ocak 2003 tarihleri arasında plevral efüzyonlu 398 hastanın klinik ve laboratuvar verilerini analiz ettik. Bu çalışma 72 TB plevral efüzyon, 147 malign plevral efüzyon, 45 parapnömonik plevral efüzyon, 18 diğer eksüdalar ve 116 transüdatif plevral efüzyon içeriyordu. Altta yatan hastalıklara göre pADA, sADA ve p/s ADA oranının ortalama ve standart sapma değerleri saptandı. sADA değerinde istatistiksel olarak gruplar arasında herhangi bir fark bulunmadı. pADA ve p/s ADA oranı TB plörezili hastalarda önemli derecede yüksek bulundu (ANOVA, p< 0.000). pADA için eşik değerini ROC eğrisi 35 U/L olarak buldu, buna göre TB plevral efüzyon tanısında duyarlılık %84.7, seçicilik %91.1 olarak saptandı, p/s ADA oranı için eşik değeri 1.4 olarak bulundu, buna göre de, duyarlılık ve seçicilik sırasıyla %80.6 ve %84.4 idi. Bizim verilerimizle elde edilen ROC eğrisinde pADA için en iyi eşik değerini 35 U/L olarak belirledik. TB plevral efüzyonun ayrımında p/s ADA oranı tek başına pADA değerinden daha üstün değildi, p/s ADA ve pADA birlikte değerlendirildiğinde tek başına pADA'dan daha yüksek duyarlılığa sahipti.
