Comparison of the effects of combined and general anesthesia on thyroid hormones levels ın pulmonary resection

Abstract

Aim: To compare the effects of combined anesthesia (combination of thoracic epidural and general anesthesia) and general anesthesia on thyroid hormones in cases that underwent pulmonary resection. Material and Methods: The cases that scheduled for elective pulmonary resection were included in this study with the consent of our hospital's ethical committee. In group I (n= 15) the combination of thoracic epidural anesthesia (TEA) and total intravenous anesthesia (TIVA); in gfbup 2 (n= 15) TIVA was administered. In the operation room ECG, SpO and in-2 vasive arterial pressure were monitored. After epidural catheter was inserted at the thoracic 7-8 or 8-9 intervertebral spa-ce, 6-10 mL bupivacaine %0.5 was given epidurally to the cases in the combined anesthesia group. In all of the cases, anesthesia was induced with 3 mg midazolam, 2 |ag/kg fentanyl, 2 mg/kg propofol and 0.1 mg/kg vecuronium and double-lumen tube was placed. In the maintanence of anesthesia; 6-7 mg/kg/hour propofol, fentanyl (1 ng/kg) and vecuronium (0.03 mg/kg) were used. Additionally 6 mL/hour bupivacaine 0.375% was given via epidural kateter in group 1. The levels of plasma thyroid hormones (sT , sT ve TSH) were determined in the biochemistry laboratory of our hospital.Results: By means of TSH and sT levels the difference between two groups was not statically significant (p> 0.05). In group I decrease in the plasma level of sT at third minute of extubation and the day after operation was statically significant compared to group 2 (p< 0.05). Conclusion: In our opinion, the plasma levels of TSH ve sT is not affected by the method of anesthesia. On the other hand, because of the significant decrease in the level of sT in combined anesthesia group we thought that TEA could have affected thyroid functions.

Amaç: Akciğer rezeksiyonu yapılan olgularda kombine anestezi [torakal epidural anestezi (TEA)-genel anestezi kombinasyonu] ile genel anestezinin tiroid hormonları üzerine etkilerini karşılaştırmak. Gereç ve Yöntem: Elektif akciğer rezeksiyonu planlanan olgular hastanemiz etik kurulunun onayıyla çalışmaya alındı. Grup l'de (n= 15) TEA-total intravenöz anestezi (TİVA) kombinasyonu; grup 2'de (n= 15) ise TİVA uygulandı. Operasyon odasında elektrokardiyografi, SpO ve invaziv arteryel basınç monitörize edildi. Kombine anestezi grubundaki olgulara to-2 rakal 7-8 veya 8-9 aralığından epidural kateter yerleştirilip, kateterden 6-10 mL %0.5 bupivakain uygulandı. Tüm olgularda anestezi indüksiyonunda 3 mg midazolam, 2 fxg/kg fentanil, 2 mg/kg propofol, 0.1 mg/kg vekuronyum uygulanıp,çift lümenli tüp yerleştirildi. Anestezi idamesinde 6-7 mg/kg/saat propofol ile fentanil (1 ng/kg) ve vekuronyum (0.03 mg/kg) bolusları kullanıldı. Ek olarak grup 1 'deki olgulara epidural yolla 6 mL/saat %0.375 bupivakain verildi. Plazma tiroid hormon düzeyleri [sT3, sT4 ve tiroid stimülan hormon (TSH)] hastanemiz biyokimya laboratuvarında analiz edildi. Sonuçlar: iki grup arasında plazma TSH ve sT4 düzeylerindeki fark istatistiksel olarak anlamlı bulunmadı (p> 0.05). Plazma sT3 düzeylerinde grup l'de ekstübasyonun üçüncü dakikasındaki ve postoperatif birinci gündeki düşme diğer gruba göre istatistiksel olarak anlamlıydı (p< 0.05). Yorum: Plazma TSH ve sT4 düzeylerinin kullanılan anestezi yönteminden etkilenmediği kanısındayız. Buna karşın, kombine anestezi grubunda sT3 düzeyinin daha anlamlı bir şekilde düşmesi, TEA'nın tiroid fonksiyonlarını etkileyebileceğini düşündürmektedir.
