A case of pulmonary arteriovenous malformation

Abstract
Pulmonary arteriovenous malformation is generally a congenital lesion that results from an abnormal capillary development. It may be asymptomatic, but exertional dyspnea, palpitations and easy fatiquability may also be seen. For diagnosis computed tomography of thorax and angiography are useful. Surgery or embolic obliteration should be performed for treatment, especially when it is associated with Rendu-Osler-Weber disease. A 28-year-old female with no complaints was found out to have a pulmonary mass on chest X-rays taken for a job application. After the detailed investigation of the lesion, it was diagnosed to be a pulmonary arteriovenous malformation. The lesion was excised by a wedge resection. We present the case together with the literature knowledge, because the patient was asymptomatic, there was widespread continuous murmur on physical examination and the lesion was suitable for surgical resection.
Özet
Pulmoner arteriyovenöz malformasyonlar (PAVM) anormal kapiller gelişimden kaynaklanan, çoğunlukla konjenital lezyonlardır. Asemptomatik olabilecekleri gibi egzersiz dispnesi, çarpıntı ve erken yorulma da görülebilir. Teşhiste toraks bilgisayarlı tomografisi ve anjiyografiden faydalanılır. Tedavide, özellikle "Rendu-Osler-Weber" hastalığı ile birlikte ise cerrahi veya embolik obliterasyon uygulanmalıdır. Yirmisekiz yaşında, hiçbir şikayeti olmayan bayan hastanın, bir iş başvurusu sırasında çekilen akciğer grafisinde tesadüfen tespit edilen lezyonun ileri incelemeler sonucu PAVM olduğu tespit edilerek "wedge" rezeksiyon uygulanmıştır. Olgumuz; asemptomatik olması, fizik muayenede yaygın ve devamlı üfürüm duyulması ve cerrahi tedaviye uygun PAVM olgusu olması nedeni ile literatür bilgileri gözden geçirilerek sunuldu
