Chronic eosinophilic pneumonia : A rare case

Abstract

A patient presenting with cough, dyspnea, rhinitis and weight loss for eleven years was hospitalised several times with the diagnosis of bronchial asthma, Loeffler's pneumonia, eosinophilic lung disease, Churg-Strauss syndrome and interstitial pneumonia. The patient's IgE levels and blood eosinophil counts were found as high in each hospitalisation and the lesions images of migrating origin with the predominance of upper zones were observed in chest-X rays. Since the at-tacks presented with severe wheezing, hypoxemia and dyspnea the corticosteroid therapy was initiated before the invasive prosedures. So that, the histologie examinations of several transbronchial bronchoscopic biopsies and one open lung biopsy revealed nonspesific findings. In the last hospitalisation during another attack, a transbronchial biopsy and a biopsy from the diffuse nodular lesions observed on the tracheobronchial tree, were taken by bronchoscopy. The histopatological examination of the biopsies revealed findings in consistent with chronic eosinophilic pneumonia.

Özet

Onbir yıldır öksürük, nefes darlığı, burun akıntısı, kilo kaybı yakınmaları olan olgu birçok kez bronşiyal astım, "Löfler" pnömonisi, eozinofilik pnömoni, "Churg-Strauss" sendromu ve interstisyel pnömoni ön tanıları ile hastaneye yatırıldı. Yatışlarındaki PA akciğer grafilerinde yer değiştiren lezyonları ve üst lob predominansı bulunan hastanın her seferinde IgE ve eozinofil değerleri yüksekti. Hastanın ataklar sırasında ileri derecede "wheezing" ve hipoksemisi bulunduğu için invaziv tetkikler öncesi kortikosteroid tedavi başlandı. Olguda birçok kez alınan transbronşiyal biyopsi ve bir kez yapılan açık akciğer biyopsi patolojisi bu nedenle nonspesifik bulundu. En son atak sırasında yeniden yatırılan hastaya yapılan bronkoskopide, trakeobronşiyal ağaçta görülen yaygın nodüler lezyonlardan biyopsi ve ayrıca transbronşiyal biyopsi alındı. Patoloji sonucu kronik eozinofilik pnömoni ile uyumlu geldi.
