Tracheobronchial foreign body aspirations: Retrospective analysis of 261 cases

Abstract
İn spite of the developed technics in the treatment of tracheobronchial foreign body aspirations, this condition may be a life-threatening medical situation and may occur in all age groups. We have discussed our management in the tracheobronhial foreign body aspirations with the literature. Two hundred and sixty one cases who presented with the suspective diagnosis of aspirated tracheobronchial foreign bodies from january 1996 to january 2003, were analyzed retrospectively for sex, age, symptoms, radiographic findings, nature of the foreign body and the treatment modalities. Of the 261 cases who had the suspective diagnosis of aspirated foreign body; foreign body was observed in 91.2% (n= 238) of them. The locations of the aspirated foreign bodies were as follows; 47.5% (n= 113) of them in the right bronchial tree, 44.1 % (n= 105) of them in the left bronchial tree, and 8.4% (n= 10) of them in the trachea. 95.7% (n= 228) of the aspirated foreign bodies were removed by rigid bronchoscopy under general anesthesia. 10 cases undervvent thorachotomy because efıdoscopic managements were not succesful in the removal of the foreign bodies. The morbidity and mortality of the cases with aspirated foreign bodies were 4.6% (n= 12) and 0.4% (n= 1) respectively. Prompt diagnosis and treatrnent are very important for the aspirated tracheobronchial foreign bodies. Although history radiographic and clinical findings are not appropriate. Endoscopic procedure must be performed if there is a suspicion of foreign body aspiration. Rigid bronchoscopy under general anesthesia is the most effective and the safest procedure in aspirated tracheobronchial foreign bodies. Thorachotomy that should be performed promptly is necessary when the endoscopic procedure in the removal of the foreign body is not succesful.

Trakeobronşiyal yabancı cisim aspirasyonları, her yaş grubunda görülebilen, gelişmiş tedavi tekniklerinin kullanıma girme-sine rağmen hayatı tehdit edebilen bir sağlık problemidir. Bu çalışmada, trakeobronşiyal yabana cisim aspirasyonlarındaki yaklaşımımızı literatür bilgileri eşliğinde tartıştık. Ocak 1996-Ocak 2003 tarihleri arasında trakeobronşiyal yabancı cisim ön tanısı ile başvuran 261 olgunun; cinsiyet, yaş, semptomlar, radyolojik bulgular, yabancı cismin natürü ve tedavi yöntemleri retrospektif olarak incelendi. Trakeobronşiyal yabancı cisim aspirasyon ön tanısı ile başvuran 261 olgunun %91.2 (n= 238)'sinde yabancı cisim tespit edildi. Aspire edilen yabancı cisimlerin lokalizasyonları; %47.5 (n= l13)'i sağ bronşiyal sistemde, %44.1 (n= 105)'i sol bronşiyal sistemde, %8.4 (n= 20)'ü trakeada idi. Trakeobronşiyal yabancı cisimlerin %95.7 (n= 228)'si genel anestezi altında rijit bronkoskopi ile çıkarıldı. Endoskopik yaklaşımlarla yabancı cisim çıkarılamayan 10 ol-guya torakotomi uygulandı. Trakeobronşiyal yabancı cisim aspirasyonu olan olgularda morbidite oranı %4.6 (n= 12), mor-talite oranı %0.4 (n= 1)'tür. Trakeobronşiyal yabancı cisimlerin erken teşhis ve tedavisi çok önemlidir. Anamnez, radyolojik ve klinik bulgu uygun olmasa bile yabancı cisim aspirasyon şüphesinde endoskopik müdahale yapılmalıdır. Trakeobronşiyal yabancı cisim aspirasyonlarında genel anesteziyle yapılan rijit bronkoskopi en etkili ve en güvenilir yaklaşımdır. Bronkoskopik müdahale ile yabancı cisim çıkarılamayan olgulara en kısa sürede torakotomi uygulanmalıdır.
