Evaluation of intrathoracic goiter cases seen in computerized thorax tomography

Abstract

Intrathoracic goiters are rare but have to be considered as diagnostic possibility in all mediastinal masses. The aim of this study was to analyze the intrathoracic goiter and to point out the diagnostic problems. In our clinic, for a period of one year (2002-2003) 293 consecutive patients underwent thorax computerized tomography (CT) for different reasons. Intrathoracic goiter was found in 24 (8.1%) patients. The diagnostic procedures include physical examination, chest radiography, thyroid ultrasonography, thyroid scintigraphy, eventually combine with fine needle aspiration biopsy (FNAB) in indicated patients. FT3, FT4 and TSH testing were performed. Intrathoracic goitre was found in 24 (8.1%) patients. Out of 24 patients, 15 had not a definitive diagnosis of thyroid disease until this time. Eleven women and 13 men included to the study. The age of the patients ranged from 37 to 82 (mean ± SD: 62.87 ± 10.57). Twelve had lung carcinoma, 5 had chronic obstructive pulmonary disease (COPD), 2 had sarcoidosis, 1 had bronchial asthma, 1 had malignant mesothelioma, 1 had pulmonary embolism, 1 had interstitial fibrosis and 1 had lung abscess. Nine patients had cervical goiters. All of intrathoracic goiters were located in the anterosuperior mediastinum. Although compression of adjacent organs were detected in 6 patients, none of those had symptoms about the airway compression and dysphagia. FNAB was done in 7 patients regarding endocrinology consultation. None of these patients were diagnosed as thyroid carcinoma. Intrathoracic goitre was diagnosed with a rate of 8.1% in the patients who were evaluated by thorax CT and 15 of those had not a definitive diagnosis of thyroid disease before. This shows the importance of the attention of the pulmonologist to the superior mediastinum in radiological evaluation.

Her ne kadar intratorasik guatr (İTG) nadir görülüyor ise de, anterior mediastinal kitlelerin önemli bir yüzdesini oluşturmaktadır. Biz de bu nedenle çalışmamızda toraks bilgisayarlı tomografi (BT) ile tesadüfen saptadığımız İTG’li olguları analiz etmeyi ve tanısal problemlerine dikkat çekmeyi amaçladık. 2002-2003 yılları arasında kliniğimizde yatarak tedavi gören ve çeşitli nedenlerle toraks BT’si çekilen 293 olgu değerlendirildi. İTG’li olguların tiroid hastalığına ait semptomları sorgulandı. Fizik muayene ile servikal guatr olup olmadığı saptandı. Tüm olguların postero-anterior akciğer grafisi, tiroid ultrasonografi (USG)’si, tiroid sintigrafisi ve tiroid hormonları yapıldı. İnce iğne aspirasyon biyopsisi sadece endokrinoloji bölümünün gerekli gördüğü olgularda uygulandı. Toraks BT çekilen 293 olgunun sadece 24 (%8.1)’ünde İTG saptandı. Bu 24 olgunun 15’i daha önce tiroid hastalığı tanısı almamıştı. Olgularımızın yaşı 37-82 arasında (ortalama ± SS: 62.87 ± 10.57) olup, 11’i kadın 13’ü erkek idi. İTG tanısı alan 24 olgunun 12’si akciğer kanseri, beşi kronik obstrüktif akciğer hastalığı, ikisi sarkoidozis, biri bronşiyal astım, biri malign mezotelyoma, biri pulmoner tromboemboli, biri pulmoner fibrozis ve biri de akciğer apsesi tanısı almış idi. Fizik muayenede sadece dokuz olguda servikal guatr saptandı. İTG’li olgularımızın tamamında tiroid anterosüperior mediastende yerleşmişti. Trakea basısı altı olguda izlenmesine rağmen olgularımızın hiçbirisinde hava yolu obstrüksiyonu veya disfajiye ait bir semptom bulunmadı. Endokrin konsültasyonu sonrası gerekli görülen yedi olguda ince iğne aspirasyon biyopsisi yapıldı. Bu olguların hiçbirisinde tiroid karsinomu saptanmadı. Toraks BT çekilen olgularımızın %8.1’inde İTG saptandı ve bunların %62’si daha önce tiroid hastalığı tanısı almamıştı. Bu da göğüs hastalıkları doktorlarının toraksa yönelik radyolojik incelemelerde süperior mediastinuma dikkat etmelerinin ne kadar önemli olduğunu göstermektedir.
