Evaluation of 153 patients with pleural effusion

Abstract
We aim to determine clinical features and etiologic factors of pleural effusions in our clinic. We evaluated patients who presented with pleural effusion to our department between January 2001 -December 2003 prospectively. Pleural effusion was evaluated by cell count, white cell differentiation, biochemical tests and culture of tuberculosis and aerobic microorganisms and cytology. We used pleural biopsy, toracotomy, video assisted thoracoscopic surgery, transthorocal biopsy and broncoscopy for evaluation in required patients. One hundred fifty-three patients (90 men, 63 women) were included in the study. The mean age of the patients was 52.2 years, the most frequent symptom of the patients was dyspnea (45%). Eighty-five pleural effusions were located on the right side, 58 were on the left and 10 were bilateral. There were 146 (95.4%) exudates and 7 (4.6%) transudates. The most prevalant etiologic factor that we determined was malignant diseases (37%) and tuberculous pleural effusion (%21), parapneumonic effusion (8%) and empyema (8%), respectively. As a result, malignant and tuberculous pleural effusions were found more than half of (58%) all pleural effusions in our study group.

Çalışmamızın amacı; kliniğimizde plevral efüzyon nedeniyle değerlendirilen hastaların etyolojilerini ve ilgili özelliklerini araştırmaktır. Ocak 2001-Aralık 2003 tarihleri arasında plevral efüzyonlu hastalar incelendi. Plevral efüzyon; hücre sayımı, beyaz küre diferansiasyonu, biyokimyasal testler, tüberküloz ve aerobik kültür, sitoloji açısından değerlendirildi. Gereken olgulara plevral biyopsisi, torakotomi, video yardımlı torakoskopik cerrahi, transtorakal biyopsi ve bronkoskopi yapıldı. Toplam 153 plevral efüzyonlu (9O'ı erkek, 63'ü kadın) olgu çalışmaya alındı. Ortalama yaş 55.2 yıl, en sık görülen şikayet dispne (%45) idi. Plevral efüzyonların 85'i sağ, 58'i sol, 10'u da bilateral yerleşimliydi. Efüzyonların 146 (%95.4)'sı eksü-da, 7 (%4.6)'si transüda özelliğindeydi. En sık bulunan plevral efüzyon nedeni malign hastalıklar (%37) idi. Bunu tüberküloz plörezi (%21), parapnömonik efüzyon (%8) ve ampiyem (%8) izlemekteydi. Sonuç olarak; çalışma grubumuzda plevral efüzyonların yarısından fazlasını (%58) malign ve tüberküloz plevral efüzyonların oluşturduğu görüldü.
