The effect of conventional treatment and erdosteine on oxidative stress in asthma patients

Abstract

The purpose of this study was to evaluate the level of oxidative stress before and after treatment in asthma patients, and also to define whether erdosteine, a mucolytic agent with antioxidant activity, has an additional benefit to treatment. We analyzed oxidative stress by using malondialdehyde (MDA) level and antioxidant defense system by using superoxide dis-mutase (SOD) activation. We prospectively studied 41 (9 male, 32 female) consecutive mild to moderate asthma patients who were treated in our hospital between August 2003 and February 2004. Patients were divided into two groups randomly. The study was planned as double-blinded placebo confirmed study. MDA levels and SOD activity were detected in serum of patients before and after 15 day period of treatment. MDA levels at baseline and after the therapy were found 1 3.32 ± 8.43 and 11.19 ± 8.76 nmol/mL, and SOD activity were found 7.71 + 4.04 and 8.20 ± 3.60 U/mL, respectively. Although there was no significant increase in the SOD activity after the therapy, we found statistically significant reduction in the level of MDA in each group (p< 0.05). In the patients of group 1 who had taken erdostein and group 2, the differences in SOD activity at baseline and after the therapy were 0.46 ± 2.59 and 0.52 + 2.77 (p= 0.611), respectively and the differences in MDA level were 2.22 ± 5.02 and 2.03 ± 4.43 (p= 0.72), respectively. As a conclusion; we found a reduction in oxidative stress and increase in antioxidant activity with treatment in asthma patients.it is shown that;erdosteine,that was given additional to standard therapy for two weeks,did not have any significant advantage on antioxidant activity.

Çalışmamızda astım olgularında, tedavi öncesi ve sonrasında oksidatif stresin düzeyini ve bu düzeye mukolitik bir ilaç olup aynı zamanda da antioksidan aktivitesi olduğu düşünülen erdosteinin ek bir katkısı olup olmadığını araştırmayı amaçladık. Oksidatif stresi malondialdehid (MDA) düzeyi ile, antioksidan savunma sistemini de süperoksid dismutaz (SOD) aktivitesi ile değerlendirdik. Çalışmaya Ağustos 2003-Şubat 2004 tarihleri arasında hastanemize başvuran hafif ve orta derecede astımı olan 9 (%21.9)'u erkek, 32 (%78.1)'si kadın toplam 41 hasta alındı. Çalışma plasebo kontrollü çift-kör olarak yapıldı. Olguların tedavi öncesi ve sonrası 15. günde serumlarında MDA ve SOD düzeylerine bakıldı. Tedavi öncesi ve sonrası MDA düzeyleri sırasıyla 1 3.32 ± 8.43 ve 11.19 ± 8.76 nmol/mL; SOD düzeyleri 7.71 ± 4.04 ve 8.20 ± 3.60 U/mL idi. Tedavi sonrasındaki SOD düzeylerindeki artış anlamlı bulunmazken, MDA düzeylerindeki düşme istatistiksel olarak anlamlı bulundu (p< 0.05). Çalışmaya alınan olgular randomize olarak iki gruba ayrıldı. Grup 1'de (erdostein alan) tedavi öncesi ve sonrası SOD farkı 0.46 ± 2.59, grup 2'de (plasebo alan) 0.52 ± 2.77 (p= 0.611); grup 1'de MDA farkı 2.22 ± 5.02, grup 2'de 2.03 ± 4.43 (p= 0.72) olarak bulundu. Sonuç olarak; astımlı hastalarda tedavi ile oksidatif streste azalma, antioksidan aktivitede ise artış olduğu belirlendi. Tedaviye ek olarak verilen iki haftalık erdostein tedavisinin antioksidan aktivite açısından bir katkı sağlamadığı belirlendi.
