The management of childhood complicated parapneumonic effusion and empyema by conservative surgical approachs

Abstract

The aim of this study was to determine the effectiveness of the conservative surgical approaches (thoracentesis/tube thoracostomy + open drainage + appropriate antibiotics) in the treatment of childhood complicated parapneumonic pleural effusions and empyema and compare them with other theraputic maneuvers by the review of the literature. Between 1998 and 2003, 127 (mean age 4.6 years) children were referred to our clinic with diagnosis of complicated parapneumonic effusion and empyema. One hundred and thirteen (88.9%) patients were treated successfully by repeating thoracentesis (tube thoracostomy) open drainage and appropriate antibiotics. Only 14 (11%) case were gone to decortication. Tube thoracostomy mean time was 20.7 days. Because of high capacity of resorption of the thickness pleura in child¬ren, long-term results of the conservative surgical approachs are excellent, if they are applied patiently.

Özet

Bu çalışmanın amacı; çocukluk çağı komplike parapnömonik efüzyon ve ampiyemlerde konservatif cerrahi yaklaşımların (torasentez/tüp torakostomi + açık drenaj + uygun antibiyotik) etkinliğini araştırmak ve literatür bilgileri eşliğinde diğer tedavi yaklaşımlarıyla karşılaştırmaktır. 1998-2003 yılları arasında 127 (ortalama 4.6 yaş) çocuk komplike parapnömonik efüzyon ve ampiyem tanılarıyla kliniğimize refere edildi. Yüzonüç (%88.9) hasta tekrarlayan torasentezle (tüp torakostomi) açık drenaj ve uygun antibiyotik ile başarılı bir şekilde tedavi edildi. Sadece 14 (%11) olgu dekortikasyona gitti. Tüp torakostomi süresi ortalama 20.7 gün olarak belirlendi. Çocukların kalınlaşmış plevranin rezorpsiyon kabiliyeti nedeniyle sabırla uygulanan konservatif cerrahi yaklaşımların uzun dönem sonuçları mükemmel olmaktadır.
