The role of clinical scoring methods in the diagnosis of pulmonary embolism

Abstract
Aim: The study was planned to investigate the diagnostic value of two different clinical scoring methods in probability of pulmonary embolism. Material and Method: Sixty-nine hospitalized patients whose one of the differential diagnosis was pulmonary embolism were included the study. All patients were evaluated with Wells and Geneva scoring methods and low, intermediate and high probability groups were determined according to these two methods. After the diagnostic algorithm was completed the diagnostic values of clinical scoring methods were evaluated. Results: Pulmonary embolism was diagnosed in 27 (39.1%) of the 69 patients. The frequencies of pulmonary embolism in the low, intermediate and high clinical probability categories were respectively: 3.4%, 40.9% and 94.4% for the Wells model; 20.7%, 36.7% and 100% for the Geneva model. The clinical and pulmonary embolism probability increased significantly in both models. After grouping intermediate and high probability groups as one group, the sensitivity and specifity of scoring methods were respectively 96.3% and 66.7% for Wells, 77.8% and 54.8% for Geneva. Conclusion: Clinical scoring methods might be used to rule out the pulmonary embolism especially in the clinics which have limited laboratory facilities. The Wells model is more valuable than the Geneva model.

Amaç: Bu çalışma, pulmoner emboli tanısında kullanılan iki değişik klinik olasılık skorlama yönteminin tanı değerinin belirlenmesi amacıyla planlanmıştır. Gereç ve Yöntem: Pulmoner emboli ön tanısıyla kliniğimize yatırılarak araştırılmış olan 69 hastada, öncelikle Wells ve Geneva klinik skorlamaları yapılıp, her iki yönteme göre düşük, orta ve yüksek olasılık grupları belirlenmiştir. Tanı algoritmasının tamamlanmasından sonra, klinik skorlama yöntemlerinin tanısal değerleri belirlenmiştir. Sonuçlar: Çalışmaya alınan 69 olgunun 27 (%39.1)'sinde pulmoner emboli tanısı konmuştur. Düşük, orta ve yüksek olasılık gruplarındaki pulmoner emboli oranları sırasıyla; Wells yönteminde %3.4, %40.9 ve %94.4, Geneva yönteminde ise %20.7, %36.7 ve %100 olarak bulunmuştur. Her iki yöntemde de klinik olasılık arttıkça pulmoner emboli oranının da istatistiksel olarak anlamlı derecede arttığı saptanmıştır. Orta ve yüksek klinik olasılık gruplarındaki olgular tek bir grup olarak kabul edilerek yapılan gruplama sonrasında, Wells yönteminin duyarlılığı %96.3, özgüllüğü %66.7; Geneva yönteminin ise duyarlılığı %77.8, özgüllüğü %54.8 olarak bulunmuştur. Yorum: Klinik skorlama yöntemleri, laboratuvar olanaklarının kısıtlı olduğu ortamlarda özellikle pulmoner embolinin ekarte edilmesinde kullanılabilir. Wells yöntemi Geneva yöntemine göre daha değerlidir.
