Global budget as a reimbursement method in health and Turkey

Abstract

In the struggle that countries give on the way of development, healthy individuals that form a healthy nation play a key role. Financing of health care services is very important in this context and very influentaloperation of that system. In this study first health care reimbursement methods were touched upon in general. A reimbursement system is a mechanism determining how the costs of health care services provided to individuals by health care providers will be puchased by the payer organization. The working principles, strong and weak sides of these mechanisms are adressed. Second, one of these systems, the global budget is dealed. The logic of global budget defined as a general spending limit that determines the price and the quality ofservices provided is examined. In the last part of the study, the implementation areas, the regulations in these areas and the experience throughout the execution process of global budget are discussed.

Ülkelerin gelişme yolunda verdiği mücadelede sağlıklı bireyler ve bu sağlıklı bireylerin oluşturduğu sağlıklı toplumlar anahtar rol oynamaktadır. Bu bağlamda büyük önemi olan sağlık hizmetleri sistemlerinin finansmanı, o sistemin işleyişi üzerinde son derece etkilidir. Bu çalışmada öncelikle sağlık hizmetlerinde geri ödeme sistemlerine genel olarak değinilmiştir. Bir geri ödeme sistemi, sağlık hizmeti sunucularına, kişilere sundukları sağlık hizmetlerinin bedelinin, ödeyici kurum ve kuruluşça nasıl geri ödeneceğini belirleyen mekanizmadır. Bu mekanizmaların işleyiş ilkelerine, güçlü ve zayıf yönlerine değinilmiştir. Ardından bu sistemlerdenolan global bütçe ele alınmıştır. Sunulan hizmetlerin ücret ve kalitesini tayin eden genel bir harcama sınırı şeklinde tanımlanan global bütçenin mantığı irdelenmiştir. Çalışmanın son kısmında ise, global bütçeninkemizdeki uygulama alanlarına, bu alanlar içerisindeki düzenlemelere ve uygulama süreci boyunca yaşanan gelişmelere yer verilmiştir.
