Accurate and Timely Medical Biochemistry Laboratory Results and Pre-analytical Errors

Abstract
Objective: In this study, it was aimed to examine preanalytical errors in samples sent to Medical Biochemistry Laboratory of a university hospital. The secondary objective was to raise awareness that the errors made in this phase affect patient outcomes and to address the need for correct analysis request and appropriate sampling in order to obtain accurate and rapid testing results. Materials and Method: All the samples that have been accepted and rejected at medical biochemistry laboratory of the hospital throughout 18 months were examined retrospectively. The distribution of rejected samples was classified by pre-analytical error categories and working groups. The type and frequency of errors in laboratory working groups were expressed as percentages. Findings: The most common reasons for rejection were incomplete or incorrect information (80,6%) and missing parameter (42,1%) in analysis request for routine biochemistry testing. Clotted specimens for hemogram (66,9%), an insufficient amount of samples for coagulation tests (100,0%), inappropriate specimens provided with the requests for hormone tests were found to be the common causes of cancellation (57,1%). The total sample cancellation rate is 0,5% of the samples. Conclusion: Pre-analytical errors constitute the largest part of the errors occurring in the total testing process. In order to reduce these errors and ensure patient outcome safety, all units involved in the process should cooperate. It was seen that continuing education should be provided to the physicians and nurses working in the units that send samples and the personnel performing specimen transfer, as well as the people working at the laboratory. Clinical biochemistry courses should be included in undergraduate medical education, especially in the final year of medical school. It was thought that requesting the tests considering the physiological variables of the patient, taking biological variations into consideration, correct labelling, obtaining all samples correctly, avoiding hemolysis in blood samples and providing appropriate transfer conditions would reduce the errors in the pre-analysis phase. We believe that ensuring the monitoring of causes of sample rejection during the error reporting process and providing feedback through the hospital information system will contribute to the reduction of pre-analytical errors.

Giriş ve Amaç: Bu çalışmada, bir üniversite hastanesi Tıbbi Biyokimya Laboratuvarına gönderilen örneklerde preanalitik hataların analizinin yapılması amaçlanmıştır. İkincil amaç, bu evredeki hataların, hasta sonuçlarını etkilediği konusunda bir farkındalık oluşturmak, doğru tetkik istemi ve uygun numune alınmasının, doğru ve hızlı test sonucu için gerekliliğine değinmektir. Gereç ve Yöntem: Hastane tıbbi biyokimya laboratuvarında 18 ay boyunca kabul ve red edilmiş olan tüm örnekler retrospektif olarak incelenmiştir. Reddedilen örneklerin dağılımı; preanalitik hata kategorileri ve çalışma gruplarına göre sınıflandırıldı. Laboratuvar çalışma gruplarındaki hata tipi ve sıklığı yüzde olarak gösterilmiştir. Bulgular: En fazla görülen red nedeni, rutin biyokimya testleri için test isteminde eksik ya da yanlış bilgi (%80,6) ve eksik parametre (%42,1)’dir. Hemogram örneklerinde pıhtılı numune (%66,9), koagulasyon testlerinde yetersiz miktarda örnek alınması (%100,0), hormon isteklerinde uygun olmayan numune sık görülen iptal nedenleri (%57,1) olarak bulunmuştur. İptal edilen toplam numune oranı, örneklerin %0,5’idir. Sonuç: Toplam test sürecindeki hatalar içerisinde preanalitik hatalar en büyük bölümü oluşturmaktadır. Bu hataların azaltılabilmesi ve hasta sonuç güvenliğinin sağlanabilmesi için konuya dahil olan tüm birimler işbirliği içinde çalışmalıdır. Laboratuvarda görev yapan kişiler ile birlikte laboratuvara örnek gönderen birimlerde çalışan doktor, hemşire ve transferi sağlayan personele sürekli eğitim verilmesinin gerekli olduğu görülmüştür. Mezuniyet öncesi tıp eğitiminde, özellikle tıp fakültesinin son sınıfında, klinik biyokimya dersleri yer almalıdır. Tetkiklerin hastanın fizyolojik değişkenleri gözetilerek istenmesinin, biyolojik varyasyonların gözönünde bulundurulmasının, doğru etiketlemenin, tüm numunelerin doğru olarak elde edilmesinin, kan örneklerinde hemolizden kaçınılmasının ve uygun transfer koşullarının sağlanabilmesinin analiz öncesi evredeki hataları azaltacağı düşünülmüştür. Hata bildirim sürecinde numune red nedenlerini monitörize edebilmenin ve hastane enformasyon sistemi aracılığı ile geri bildirim vermenin, preanalitik hataların azaltılmasına katkıda bulunacağı düşünülmektedir.
