Endoscopic transnasal transsphenoidal surgery for pituitary tumors

Abstract
Objectives: Transseptal transsphenoidal surgery of pituitary tumors is the most commonly used technique. In this study, we report our experience in seven patients with sellar lesions in whom an endoscopic sphenoidectomy approach to the sella was used. Patients and Methods: After the anterior wall of the sphenoid sinus was opened using a 0° endoscope, we inserted. a nasal speculum in four patients with a narrow nasal cavity and used a rigid endoscope in conjunction with an operating microscope. In the remaining three patients, only rigid endoscope was used throughout the operation. Results: This approach provided both an excellent exposure of the sella and an adequate working space for the surgeon. Minimal operative pain was observed and, in some cases, hospital stay was shorter. No significant complications were encountered during the follow-up. Conclusion: The endoscopic transseptal approach to the sphenoid sinus for pituitary surgery was found to be easy and time-saving, without any septal or sublabial complications.

Amaç: Pitüiter tümörlerde transseptal transsfenoidal cerrahi en sık kullanılan cerrahi yöntemdir. Bu yazıda, sellaya endoskobik sfenoidektomi tekniği ile yaklaştığımız pitüiter tümörlü yedi hastadaki deneyimimizi sunduk.Hastalar ve Yöntemler: Sfenoid sinüsün ön duvarının 0° endoskop kullanılarak açılmasını takiben, nazal kavitesi dar olan dört olguda nazal spekulum yerleştirilerek ameliyatta endoskopla mikroskop kombinasyonu tercih edildi. Diğer üç olguda ise tüm ameliyat boyunca sadece endoskop kullanıldı.Bulgular: Bu teknik, sellaya mükemmel bir ekspozisyon yanında cerraha yeterli bir çalışma alanı da sağladı. Ameliyat sonrası ağrı minimal idi. Bazı olgularda hastanede kalış süresi kısaldı. Takip edilebilen hastaların hiçbirinde önemli bir komplikasyona rastlanmadı. Sonuç: Pitüiter tümörlerde sfenoid sinüse endoskobik teknik ile yaklaşım, septal veya sublabial komplikasyonlar olmaksızın sellaya ulaşımda kolay ve hızlı bir yöntemdir.
