Magnetic resonance cholangiography in obstructive jaundice

Abstract
Objectives: We evaluated the effectiveness of HASTE magnetic resonance cholangiography (MRC) in detecting the level and cause of obstruction of dilated biliary ducts. Patients and Methods: Thirty-seven patients (23 men, 14 women; age range 31-82 years) with an initial diagnosis of obstructive jaundice were evaluated with MRC obtained in axial, coronal and oblique planes. Additional modalities used were computed tomography (n=19), conventional magnetic resonance imaging (n=14), and endoscopic retrograde cholangiopancreatography (ERCP) (n=2). Findings obtained by MRC were confirmed with those from ERCP and/or surgery. Results: Magnetic resonance cholangiography revealed choledocolithiasis in 21, perforated hydatid cyst in 2, and malignant obstruction in 13 cases. Biliary dilatation was shown in all patients in whom bile duct dilatation was documented using other imaging modalities. The cause of obstruction could not be determined in only one patient, in whom intraoperative cholangiography showed a benign structure. Surgical diagnosis was negative in one patient in whom MRC showed a bile calculus. A strong correlation was found between the findings of MRC and surgery with respect to the presence and the level of obstruction (Cohen's kappa: 0.95; p<.000). The sensitivity of the MRC was 100% in showing biliary dilatation and 97.2% in the determination of the level and the cause of obstruction. Conclusion: Magnetic resonance cholangiography is an easy, noninvasive, and reliable technique, providing relevant preoperative findings when combined with those obtained from other radiological modalities.

Amaç: Biliyer obstrüksiyonda HASTE sekanslı manyetik rezonans kolanjiyografinin (MRK) safra yolları dilatasyonu ile tıkanma düzeyinin ve nedeninin gösterilmesindeki etkinliği araştırıldı. Hastalar ve Yöntemler: Tıkanma ikteri öntanısı konan 37 hastada (23 erkek, 14 kadın; yaş dağılımı 31-82) aksiyel, koronal ve oblik sagital planlarda ince kesit HASTE sekansı ile intra ve ekstrahepatik safra yolları incelendi. Olguların 19'u bilgisayarlı tomografi, 14'ü konvansiyonel manyetik rezonans görüntüleme, ikisi ise endoskopik retrograd kolanjiyopankreatikografi (ERKP) ile de değerlendirildi. Manyetik rezonans kolanjiyografi bulguları ERKP ve/veya cerrahi girişimle doğrulandı. Bulgular: Manyetik rezonans kolanjiyografi ile 21 olguda koledoktataş, iki olguda safra yollarına perfore kist hidatik, 13 olguda malign obstrüksiyon saptandı. Biliyer genişleme, diğer görüntüleme yöntemleri ile safra yollarında dilatasyon saptanan hastaların tümünde MRK ile de gösterildi. Obstrüksiyonun nedeninin belirlenemediği tek olgu perioperatif kolanjiyografi ile benign yapı olarak değerlendirilirken, MRK ile koledokta taş saptanan bir olgu ameliyatta negatif tanı aldı. Obstrüksiyon varlığı ve seviyesinin saptanması açısından MRK ve cerrahi bulgular arasında yüksek uyum bulundu (Cohen kappa: 0.95; p<.000). Biliyer sistem dilatasyonunun gösterilmesinde ve tıkanma düzeyinin ve nedeninin belirlenmesindeki duyarlılık sırasıyla %100 ve %97.2 bulundu. Sonuç: Biliyer sistem obstrüksiyonlarında MRK'nin kolay uygulanabilir, noninvaziv ve güvenilir bir yöntem olduğu, diğer radyolojik yöntemlerle korele edildiğinde cerrahi öncesi tanıya önemli katkıda bulunabileceği sonucuna varıldı.
