Abstract

Objectives: We compared sequential and staged bilateral total knee replacements performed in patients with gonarthrosis in terms of functional results, hospitalization, hospital cost, and complications. Patients and Methods: Fifty-six knees of 28 patients (4 men, 24 women) who underwent bilateral total knee replacements were retrospectively evaluated. Sequential approach (group I:14 patients; mean age 69 years) and staged approach with a mean interval of six months (group II: 14 patients; mean age 67 years) were performed for bilateral total knee replacements. Functional results were evaluated using the system proposed by the Knee Society. Mean follow-ups were 31.7 and 34.7 months in groups I and II, respectively. Results: No significant differences were found between the two groups with respect to knee and functional results, with postoperative increases by 31.3 and 31.7 points in group I and 31.4 and 31.3 points in group II, respectively (p>0.05). Significant differences were observed in favor of group I in the duration of hospitalization (14 days vs 25 days) and mean hospital cost (p<0.01). The occurrence of surgery-associated or systemic complications did not differ significantly. Conclusion: Sequential bilateral total knee replacements seem to be more convenient in ASA 1 and 2 patients with bilateral gonarthrosis.

Özet

Amaç: Bilateral gonartrozlu hastalarda tek seansta veya farklı yatışlarda uygulanan iki taraflı total diz protezi fonksiyonel sonuç, hastanede kalış süresi, hastane maliyeti ve komplikasyonlar açısından karşılaştırıldı. Hastalar ve Yöntemler: Total diz protezi uygulanan 28 hastanın (4 erkek, 24 kadın) 56 dizi retrospektif olarak değerlendirildi. On dört hastanın 28 dizine aynı seansta (grup I; yaş ort. 69; dağılım 54-77), 14 hastanın 28 dizine ise farklı yatışlarda ve ortalama altı ay arayla (grup II; yaş ort. 67; dağılım 43-78) iki taraflı total diz protezi uygulandı. Fonksiyonel sonuçlar, Diz Cemiyeti'nin (Knee Society) diz ve fonksiyon skorlama sistemi ile değerlendirildi. Ortalama takip süresi grup I'de 31.7 ay, grup II'de 34.7 ay idi. Bulgular: Diz ve fonksiyon skorlarında gruplar arasında anlamlı fark saptanmadı (p>0.05); grup I'de sırasıyla ortalama 31.3 ve 31.7 puanlık, grup II'de ise 31.4 ve 31.3 puanlık artış sağlandı. Hastanede kalış süresi (grup I'de ortalama 14 gün, grup II'de ortalama 25 gün) ve hastane maliyeti yönünden grup I lehine anlamlı derecede fark bulundu (p<0.01). Cerrahi ve sistemik komplikasyonlar yönünden gruplar arasında anlamlı farklılık gözlenmedi. Sonuç: ASA 1 ve 2 bilateral gonartrozlu hastalarda tek seansta iki taraflı total diz protezi yapılmasının uygun bir yaklaşım olduğu sonucuna varıldı.
