The role of CT-arthrography in the diagnosis of patellofemoral chondritis

Abstract
Objectives: To evaluate the effectiveness of computed tomography-arthrography in the diagnosis of patellofemoral chondritis. Patients and Methods: The study included 30 patients (21 males, 9 females; mean age 43 years; range 25-70 years) who presented with complaints of anterior knee pain and had preliminary diagnoses of patellofemoral chondritis. Double contrast CT-arthrography was performed in all patients, followed by arthroscopic surgery for precise diagnosis and management within a week after the procedure. Then, findings of CT-arthrography and arthroscopy were compared. Results: CT-arthrography showed 36 chondral lesions in 28 patients (93%), whereas arthroscopy revealed 41 lesions in 27 patients (90%). Four (14%) of the 29- chondral lesions diagnosed with arthroscopy could not be detected with CT-arthrography. On the other hand, of 25 chondral lesions observed both by CT-arthrography and arthroscopy, the stages of 18 lesions (72%) were found consistent. Conclusion: CT-arthrography enables the surgeon to identify and diagnose both patellofemoral pathologies and other intraarticular lesions. It has an effective role in deciding arthroscopic treatment of patients with patellofemoral chondritis.

Amaç: Patellofemoral kondropatisi olan hastalarda tanı koymada bilgisayarlı tomografi-artrografinin etkinliğini irdelemek. Hastalar ve Yöntemler: Çalışmaya ön diz ağrısı şikayeti ile kliniğimize başvuran ve patellofemoral kondropati öntanısı alan 30 hasta (21 erkek, 9 kadın; ort. yaş 43; dağılım 25-70) alındı. Tüm hastalarda çift kontrastlı diz BT-artrografi incelemesi yapıldı. Hastalar bu incelemeden sonra, en geç bir hafta içinde tanı ve tedavi amacıyla artroskopik cerrahi yöntemle değerlendirildi. Daha sonra BT-artrografi ve artroskopi bulguları karşılaştırıldı. Bulgular: BT-artrografide 28 hastada (%93) 36 değişik yerde kondropatik değişiklik saptandı. Artroskopide ise, 30 hastanın 27'sinde (%90) 41 lokalizasyonda lezyon görüldü. Artroskopide kondropati saptanan 29 lokalizasyondan dördü (%14) BT-artrografide izlenemedi. BT-artrografi ve artroskopide ortak olarak gözlenen 25 lokalizasyondaki kondropatik değişikliklerin evreleri yalnızca 18'inde (%72) uyumlu bulundu. Sonuç: BT-artrografinin patellofemoral eklem patolojilerinde gerek veri zenginliği gerekse komşu patolojileri işaret edebilmesi açısından ortopedisti yönlendirici bir görev üstlendiği ve artroskopik cerrahi tedaviye karar vermede önemli bir role sahip olduğu sonucuna varıldı.
