A comparative analysis of transurethral electro-vaporization and transurethral resection of the prostate in the treatment of benign prostatic hyperplasia

Abstract
Objectives: To compare the results of transurethral resection of the prostate (TURP) and transurethral electrovaporization (TUVP) in terms of operative and postoperative morbidity. Patients and Methods: Patients with benign prostatic hyperplasia (BPH) were randomly assigned to TUVP (n=37; mean age 63 years; range 50 to 82 years) or TURP (n=35; mean age 64 years; range 49 to 78 years). All patients were evaluated preoperatively and in the postoperative sixth month with respect to urinary symptomatology and urinary flow rates; postoperative bleeding, duration of catheterization, potency, and incidence of retrograde ejaculation were evaluated as well. Results: Preoperative values of IPSS, Qmaxi residual urine, and prostate volume did not differ significantly between the two treatment groups. Improvements obtained in IPSS scores and urinary flow rates (Qmax) six months after surgery were not significantly different, either. However, the results were significantly different in favor of TUVP with respect to postoperative blood hemoglobin and serum sodium levels, and the duration of operation and catheterization. Conclusion: Our sixth month follow-up results suggest that TUVP is as effective as TURP in the treatment of BPH, with additional advantages as shorter operation and catheterization times, lower postoperative morbidi-ty, and an easy learning curve.

Amaç: Benign prostat hiperplazisinin tedavisinde transüretral rezeksiyonla (TURP) transüretral elektrovaporizasyonun (TUVP) sonuçlarını ameliyat sırasındaki ve sonrasındaki morbitidite açısından karşılaştırmak. Hastalar ve Yöntemler: Benign prostat hiperplazisi nedeniyle cerrahi girişim planlanan 37 hastaya (ört. yaş 63; dağılım 50-82) TUVP, 35 hastaya (ort. yaş 64; dağılım 49-78) TURP uygulandı. Hastalar ameliyat öncesi dönemde ve ameliyat sonrası altıncı ayda üriner semptomatoloji ve idrar akım hızı ölçümlerine göre; ayrıca, ameliyat sonrası dönemde kanama, sonda kalım süresi, potens durumu ve retrograd ejekulasyon sıklığı gibi parametrelerle değerlendirildi. Bulgular: İki gruptaki hastaların ameliyat öncesi IPSS, Qmax, rezidüel idrar ve prostat hacmi değerleri benzer düzeylerde bulundu. Ameliyattan altı ay sonra, IPSS ve idrar akım hızı (Qmax) bakımından iki tedavi grubunda sağlanan iyileşmeler gruplar arasında istatistiksel açıdan anlamlı farklılık göstermedi. Buna karşın, kan hemoglobin ve serum sodyum düzeylerinde ameliyat öncesine göre ve ameliyat süresi ve sonda kalış süreleri bakımından TURP aleyhine anlamlı farklılıklar saptandı (p<0.05). Sonuç: Altıncı aydaki sonuçlarımıza göre TUVP, BPH'nin cerrahi tedavisinde en az TURP kadar etkilidir. Ayrıca TURP'a göre daha az invaziv olması, kısa kateterizasyon süresi, hızlı iyileşme süresi ve kolay uygulanması da avantajları arasındadır.
