Ureteroscopic treatment of lower ureteral calculi: A multiaspect evaluation

Abstract
Objectives: We evaluated the ureteroscopy procedures in patients with lower ureteral stones without any clinical manifestations of obstruction in the distal urethra. Patients and Methods: A total of 80 patients (52 males, 28 females; mean age 44 years; range 17-69 years) with 84 renoureteral units (RUUs) were treated by ureteroscopy in the last five years. Statistical evaluation was made with the use of the Pearson correlation test. Results: The mean stone size was 1.2 cm (range 0.4 to 2 cm). While 26 stones were removed by ureteroscopy alone (31%), additional procedures including electrohydraulic energy, basket application, pneumatic or ultrason-ic lithotripsy were applied in 58 (69%) stones. The mean operation time was 35 minutes (range 20 to 54 minutes). The procedure was completely successful in 80 RUUs (95%), with 70 RUUs (83%) following the first attempt, four RUUs (5%) following the second ureteroscopy, and six RUUs (7%) following the first ureteroscopy and ureteral dilatation. The results were unsuccessful in four RUUs (5%) and ureterolithotomy was performed. The mean hospitalization time was 36 hours. No significant correla-tion was found between the stone size and the endoscopic intervention (r=16, p>0.05), whereas the stone site and the adhesion of the stone to the ureteral epithelium were correlated both with the operation time and the amount of irrigation fluid used (r:39, p<0.05 and r=76, p<0.05, respectively). Conclusion: Our results indicate that ureteroscopy should be the first choice of treatment in selected cases with lower ureteral calculi.

Amaç: Klinik olarak üreter distal kısmında anatomik tıkanıklığın bulunmadığı üreter alt uç taşlı olgularda üreterorenoskopi (URS) girişimini ve sonuçlarımızı değerlendirdik. Hastalar ve Yöntemler: Son beş yıl içinde 80 hastada (52 erkek, 28 kadın; ort. yaş 44; dağılım 17-69) üreter orta ve alt uç taşları bulunan 84 reno-üreteral ünite (RÜÜ) Üreteroskopik yöntemle tedavi edildi, istatistiksel değerlendirmede Pearson korelasyon testi uygulandı. Bulgular: Taş boyutu ortalama 1.2 cm (0.4-2 cm) bulundu. Yirmi altı RÜÜ'de (%31) yalnız URS yöntemi ile taş ekstrakte edilirken, 58 RÜÜ'de (%69) elektrohidrolik enerji, basket uygulama, pnömatik ve ultrasonik litotripsi gibi ek yöntemlere başvuruldu. Ameliyat süresi ortalaması 35 dakika (20-54 dakika) idi. Toplam 80 RÜÜ'de (%95) başarılı sonuç alındı; birinci URS sonrası 70 RÜÜ'de (%83), ikinci URS sonrası dört RÜÜ'de (%5) ve birinci URS sonrası üreteral dilatasyonu takiben altı RÜÜ'de (%7) taştan temizlenme gerçekleşti. Başarısız sonuç alınan dört RÜÜ'de (%5) üreterolitotomi uygulandı. Hastanede kalış süresi ortalaması 36 saat bulundu. Taş hacmi ile yardımcı endoskopik girişim uygulanması arasında korelasyon saptanmazken (r=16, p>0.05), taş hacmi ile ameliyat süresi ve ameliyatta kullanılan sıvı miktarı arasında anlamlı derecede korelasyon saptandı (r=39, p<0.05). Ayrıca taşın mukozaya adezyonu ile cerrahi süresi, kullanılan sıvı miktarı arasında pozitif korelasyon bulundu (r=76, p<0.05). Sonuç: Elde ettiğimiz sonuçlar, seçilmiş olgularda alt üreter taşlarının tedavisinde üreteroskopinin ilk seçenek olması gerektiğini göstermektedir.
