The effects of remifentanil anesthesia with desflurane or propofol on hemodynamics and recovery in vertebral disc surgery

Abstract

Objectives: We investigated the effects of remifentanil anesthesia with desflurane or propofol on hemodynamics and recovery in vertebral surgery. Patients and Methods: Forty ASA l-ll patients undergoing elective vertebral surgery were randomiy assigned to two groups equal in number. Anesthesia was induced with the use of 1 $\mu$g/kg remifentanil, 2 mg/kg propofol, and 0.1 mg/kg vecuronium. Maintenance of anesthesia was obtained with 3% desflurane in one group, and with 3 mg/kg/h propofol in the other. Both groups received remifentanil infusion at 0.25 $\mu$g/kg/min from the onset of surgical incision. Systolic, diastolic, and the mean arterial pressures, heart rate, and peripheral oxygen saturation were monitored during anesthesia. Recovery scores were calculated in both groups. Results: No significant differences were found between the two groups with respect to systolic, diastolic, and the mean arterial pressures, heart rate, and peripheral oxygen saturation (p>0.05). Heart rates showed significant decreases in the propofol group 15 minutes and 30 minutes after surgical incision (p<0.05 and p<0.01, respectively). Recovery times and recovery scores did not differ significantly (p>0.05). However, nausia and vomiting occurred significantly less in the propofol group (15% and 5% versus 80% and 60%, respectively) (p<0.01). Conclusion: Our data showed that both drugs provided early recovery in vertebral surgery without hemodynamic adverse events.

Özet

Amaç: Vertebral disk cerrahisinde propofol veya desfluranın remifentanil ile kullanıldığında hemodinamik parametreler ve uyanma sürecine etkileri araştırıldı. Hastalar ve Yöntemler: Rastgele iki gruba ayrılan 40 hastada, indüksiyon için 1 $\mu$g/kg remifentanil, 2 mg/kg propofol ve 0.1 mg/kg vekuronyum uygulanarak entübasyon sağlandı. Anestezi idamesi bir grupta %3 konsantrasyonda desfluran, diğerinde 3 mg/kg/saat propofol infüzyonu ile sağlandı, iki grupta da cerrahi insizyonla birlikte 0.25 $\mu$g/kg/dk hızında remifentanil infüzyonuna başlandı. Anestezi süresince olguların kalp atım hızı, sistolik, diyastolik ve ortalama arter basınçları ile periferik oksijen satürasyonu değerleri; anestezinin sonlandırılmasmdan sonra ise anestezi sonrası derlenme skorları kaydedildi. Bulgular: Gruplar arasında sistolik, diyastolik ve ortalama arter basınçları ile periferik oksijen satürasyonu açısından bir fark bulunmadı (p>0.05). Cerrahi insizyon sonrası 15. dakikada propofol grubunda kalp atım hızının desfluran grubuna göre anlamlı düşüş gösterdiği görüldü (p 0.05). Bulantı ve kusma, propofol grubunda (%15 ve %5) desfluran grubuna (%80 ve %60) göre anlamlı düzeyde daha az saptandı (p<0.01). Sonuç: Bulgularımız, her iki ilacın da hemodinamik yan etkilere yol açmadan, vertebral disk cerrahisinde erken derlenme sağladığını gösterdi.
