A case of traumatic diaphragmatic rupture

Abstract

Diaphragmatic injuries may occur after abdominothoracic traumas. They are associated with increased morbidity and even mortality when they go unnoticed; therefore, they should be borne in mind following a traumatic event. A forty-eight-year-old male patient was admitted to our emergency department after a traffic accident. On initial examination, no pathology was observed. On the third day of hospitalization, a control chest X-ray film shovved colonic gas in the left hemithorax, which was then confirmed by computed tomography. The patient underwent laparotomy and the diaphragmatic rupture was primarily repaired. He was discharged home on the 15th day without any complications.

Diyafram yaralanmaları abdiminotorakal yaralanmalardan sonra oluşabilir. Tanı konmadığı durumlarda mortalite ve morbidite görüldüğünden travmalardan sonra mutlaka akılda bulundurulması gerekir. Trafik kazası geçiren 48 yaşındaki erkek hastanın ilk muayenesinde patoloji saptanmadı. Multitravma nedeniyle takibe alınan olgunun hastaneye yatırılmasının üçüncü gününde, kontrol akciğer grafisinde ve toraks bilgisayarlı tomografide sol hemitoraksta kolonik gaz görüldü. Hastaya erken dönemde tanı konulması nedeniyle laparotomi yapıldı. Diyaframdaki yırtık primer onarıldı. Ameliyat sonrasında sorunu olmayan hasta 15. günde taburcu edildi.
