Our clinical experience with the treatment of cancer of the esophagus and cardia

Abstract

Objectives: We evaluated patients who were treated for cancer of the esophagus and cardia within a 12year period. Patients and Methods: The study included 41 patients (20 females, 21 males; mean age 58 years; range 23 to 81 years) who underwent treatment for cancer of the esophagus and cardia. The patients were evaluated with regard to etiology, clinical findings, geographic distribution. of cases, tümör localization, surgical procedures, histopathologic findings, staging, mortality, morbidity, and survival. Results: A history of consumption of höt tea and at least a pack of cigarettes was obtained in 92.6% and 48.7%, respectively. Twenty-six patients (63.4%) were from the northeastern part of Turkey. The most common complaints were difficulty in swallowing (70.7%) and weight loss (58.5%). The tumors were in the midesophagus (n=5; 12.2%), lower esophagus (n=14; 34.2%), and the cardia (n=22; 53.6%). Thirty-eight patients (92.6%) had advanced disease, and 33 patients (80.4%) had resectable tumors. Tumor resection was performed through transhiatal (n=10), transthoracic (n=10), and abdominal approaches (n=13). None of the patients had neoadjuvant therapy. Morbidity and mortality rates were 36.5% and 17%, respectively. A significant difference was found betvveen the mean ages of patients who died in the postoperative period and who survived (p=0.0031). The mean survival was 23.75 months in 16 patients who were evaluable in the follow-up period. Conclusion: Although diagnosis of cancers of the esophagus and cardia at an advanced stage adversely affects the survival, surgical resection provides the best palliation.

Amaç: Kliniğimizde 12 yıllık bir süreçte özofagus ve kardiya kanseri nedeniyle tedavi edilen hastalar değerlendirildi. Hastalar ve Yöntemler: Çalışmaya özofagus ve kardiya kanseri tanısı konan 41 hasta (20 kadın, 21 erkek; ort. yaş 58; dağılım 23-81} alındı. Hastalar etyoloji, klinik bulgular, coğrafi dağılım, tümörün yerleşimi, uygulanan ameliyatlar, histopatolojik bulgular, evre, mortalite, morbidite ve yaşam süreleri açısından retrospektif olarak incelendi. Bulgular: Olguların %92.6'sında sıcak çay, %48.7'sinde günde en az bir paket sigara içme alışkanlığı vardı. Yirmi altı hasta (%63.4) Doğu Anadolu Bölgesi'nin kuzey illerindendi. En sık görülen klinik belirti yutma güçlüğü (%70.7) ve kilo kaybı (%58.5) idi. Tümör, beş has-tada (%12.2) orta özofagus, 14'ünde (%34.2) alt özofagus, 22'sinde (%53.6) kardiya yerleşimliydi. Otuz sekiz hastada (%92.6) ileri evrede tümör saptandı. Otuz üç hastada (%80.4) tümör rezektabl idi. Tümör rezeksiyonu 10 hastada transhiatal, 10'unda transtorasik, 13'ünde abdominal yoldan yapıldı. Hiçbir hastaya neoadjuvan tedavi uygulanmadı. Morbidite ve ölüm oranları sı-rasıyla %36.5 ve %17 bulundu. Rezektabl tümörü olan ve ameliyat sonrası dönemde kaybedilen hastaların yaş ortalaması 70.1, taburcu edilenlerin yaş ortalaması 53.2 idi (p=0.0031). Rezeksiyon yapılan ve izlenebilen 16 hastada ortalama sağkalım süresi 23.75 ay idi. Sonuç: Özofagus ve kardiya kanserlerinde ileri evrede tanı konması sağkalımı olumsuz etkilese de cerrahi rezeksiyon başarılı bir palyasyon sağlamaktadır.
