Multipl Retinal Arterial Embolies after Diagnostic Coronary Anjiography

Abstract

Transient ischemic attacks (0.1%), arrhythmia, local vascular problems (1.6%), vasovagal reaction (2.1%), and allergy (0.1-2%) can be seen after diagnostic coronary angiography (DCA). It is known that anterior and posterior ischemic neuropathy and central retinal artery occlusion may develop after coronary bypass surgery. Even though very few ocular complications occur due to embolism after DCA, serious problems can be experienced in diagnosis and treatment. The risk of encountering this complication is rising due to the increa- sing number of diagnostic radiological interventions and patients requiring this procedure. A 44-year-old male patient presented to our clinic with complaints of decreased visual acuity in the lefte ye since 3 weeks. The patient told us he underwent DCA 3 weeks ago with a suspicion of coronary ischemia because of chest pain, and visual loss developed after the procedure. The best corrected visual acuity with Snellen’s chart was found to be 1.0 in the right eye and 0.5 in the left eye. On the fundus examination, multiple embolism-related stenoses in the right and left retinal arterial branches and exudates, especially denser in the posterior pole of the left eye, were present. Exudates covered a large area at the posterior pole and temporal to the optic disc in the left eye and smaller areas in the right eye. After ophthalmic examination and diagnostic tests multiple embolies due to DCA was the diagnosis.

Özet

Tanısal koroner anjiografi (TKA) sonrası inme, geçici iskemik ataklar (%0.1), aritmi, lokal damarsal problemler (%1.6), vazovagal re- aksiyon (%2.1), ve alerji (%0.1-2) görülebilir. Koroner bypass cerrahisi sonrası anterior ve posterior iskemik nöropati, santral retinal ar- ter tıkanıklığı gelişebildiği bilinmektedir. 3 Ancak TKA sonrası emboliye bağlı oküler komplikasyonların çok az görülmesi nedeniyle bu konuda gerek teşhis gerek tedavide ciddi sıkıntılar yaşanabilmektedir. Tanısal radyolojik girişim sayısının ve bu girişime ihtiyaç duyan hasta sayısının gün geçtikçe artması nedeniyle bu komplikasyonla karşılaşma riski artmaktadır. Kırk dört yaşında erkek hasta sol gözde 3 hafta önce başlayan görme azalması şikâyeti ile kliniğimize başvurdu. Hastadan kendisine üç hafta önce göğüs ağrısı ile başvurduğu dış merkezde koroner iskemi ön tanısı ile tanısal koroner anjiografi uygulandığı ve görme kaybının işlem sonrasında geliştiği öğrenildi. Snellen eşeli ile en iyi düzeltilmiş görme keskinliği sağ gözde 1.0, sol gözde ise 0,5 düzeyinde bulundu. Fundus muayenesinde sağ ve sol gözde retina arter dalcıklarında çoklu embolilere bağlı darlıklar ve özellikle sol gözde arka kutupta daha yoğun olmak üzere eksu- dalar mevcuttu. Eksudalar sağ gözde optik disk temporalinde ve ufak iken, sol gözde optik disk temporali ve fovea alt komşuluğunda olup oldukça geniş bir alanı kaplamaktaydı. Hastanın oftalmolojik muayenesi ve tanısal tetkileri yapıldıktan sonra TKA sonrası çoklu emboli tanısı konuldu.
