The Prognostic Factors Affecting Anatomical and Functional Success in the Management of the Retinal Detachment with Giant Retinal Tears

Abstract
Purpose: To evaluate the factors affecting anatomical and functional success in the surgical management of the retinal detachment with giant retinal tears (GRT). Materials and methods: In this study, medical records of 75 eyes of 75 patients who surgically treated due to GRT were reviewed retrospe- ctively. Pars plana vitrectomy (PPV) was performed in 53 patients and PPV + scleral buckling in 22 patients. Results: Patients were divided into pairwise groups considering the presence of preoperative PVR, trauma history, macular detachment and recurrence. Final best-corrected visual acuity (BCVA) levels and postoperative visual gain were signifi cantly lower in the groups with these factors. The type of surgical procedure has no signifi cant effect on fi nal BCVA and postoperative visual gain. There were statistically sig- nifi cant negative correlations between the GRT size, mean duration of complaints and the levels fi nal BCVA and postoperative visual gain. Trauma history, PVR, macular detachment, greater tear size were the prognostic factors associated with poor fi nal BCVA and visual gain (th- ree-way ANOVA). PVR, macular detachment, greater tear size were found to be risk factors for the recurrence (logistic regression analysis). Conclusions: Mean duration of complaints, tear size, trauma history, PVR, macular involvement were considered as the prognostic factors affecting the success of surgical treatment of the retinal detachment with GRT.

Amaç: Dev yırtıklı retina dekolmanı (DYRD) tanısıyla cerrahi tedavi uygulanan hastalarda anatomik ve fonksiyonel başarıyı etkileyen prognostik faktörlerin incelenmesi. Gereç ve Yöntem: Bu çalışma kapsamında DYRD tanısıyla cerrahi tedavi uygulanan 75 hastanın 75 gözüne ait medikal kayıtlar geriye dönük olarak incelendi. Cerrahi yaklaşımda 53 hastaya pars plana vitrektomi (PPV), 22 hastaya ise PPV + skleral çökertme (SÇ) cerrahisi uygulandı. Bulgular: Hastalar cerrahi öncesi proliferatif vitreoretinopati (PVR), travma hikâyesi, makula tutulumu ve cerrahi sonrası nüks gelişimi dikkate alınarak ayrı ayrı ikişerli gruplara ayrıldı. Bu faktörlerin bulunduğu hasta gruplarındaki sonuç en iyi düzeltilmiş görme keskinliği (EİDGK) ve görme artışı düzeyleri istatistiksel anlamlı düzeyde düşük idi. Seçilen cerrahi yöntemin sonuç EİDGK ve görme artışı düzeyi üzerine istatistiksel anlamlı bir etkisinin olmadığı görüldü. Yırtık büyüklüğü ve şikâyet süresi ile sonuç EİDGK ve görme artışı düzeyleri arasında istatistiksel anlamlı düzeyde negatif korelasyon vardı. Travma öyküsü, PVR, makula tutulumu, yırtık büyüklüğü sonuç EİDGK ve görme artışı üzerine etkili kötü prognostik faktörler idi (üç yönlü Anova testi). Ayrıca PVR, makula tutulumu, yırtık büyüklüğünün nüks gelişimi için risk faktörleri olduğu tespit edildi (lojistik regresyon analizi). Sonuç: Şikâyet süresi, yırtık boyutu, travma öyküsü, cerrahi öncesi PVR mevcudiyeti, makula tutulumlu DYRD cerrahi tedavi başarısını etkileyen prognostik faktörler olduğu düşünüldü.
