Our results of 23 gauge transconjunctival pars plana vitrectomy in cases of acute endophthalmitis developing after cataract surgery

Abstract

Purpose: To evaluate the outcomes of 23 gauge (G) pars plana vitrectomy (PPV) in cases of acute endophthalmitis developing after cataract surgery. Materials and Methods: The medical records of cases who were performed 23 G PPV for acute endophthalmitis which occured after cataract surgery are evaluated retrospectively. The clinical features, visual acuity and the complications were noted. Results: Twenty-one postoperative acute endophthalmitis cases with a mean age of 71.4 (48-84) years were included in this study. All cases were performed 23 G PPV with intraocular antibiotic injection regardless of initial visual acuity. The mean follow-up time was 12.9 (6-36 months). Culture results were achived as positive only in 5 (23.8%) cases. The presenting visual acuity was hand motion, light perception and projection or worse in all cases. In 6 (28.6%) cases second vitrectomy surgery was performed after 72 hours because of inefficient results. Retinal detachment surgery with silicone oil was performed in one case because of postoperative retinal detachment. In one case inflammation was not controlled and evisceration was performed. At the final follow-up visit the visual acuity was between counting fingers and 0.05 in 14 cases (66.7%), and between 0.1 and better in 6 cases (28.6%). Conclusion: In our cases with acute endophthalmitis that developed after cataract surgery, we determine that 23 G sutureless PPV performed as an initial treatment was safe and effective in increasing the visual acuity. Further studies with longer follow-up periods are needed to evaluate the safety and the effectiveness of the procedure.

Amaç: Katarakt cerrahisi sonrası gelişen akut endoftalmi olgularında 23 gauge (G) pars plana vitrektominin (PPV) etkinliğinin değerlendirilmesi. Gereç ve Yöntem: Katarakt cerrahisi sonrası gelişen akut endoftalmi nedeni ile 23 G PPV uygulanan olgularının tıbbi kayıtları geriye dönük olarak incelendi. Olguların klinik özelikleri, görme keskinlikleri ve komplikasyonlar kaydedildi. Bulgular: Yaş ortalaması 71.4 (48-84) yıl olan 21 ameliyat sonrası akut endoftalmi olgusu çalışmaya dahil edildi. Tüm olgularda başlangıç görme düzeyine bakılmaksızın intraoküler antibiyotik enjeksiyonu ile birlikte 23 G PPV uygulandı. Ortalama izlem süresi 12.9 (6-36) aydı. Sadece 5 olguda (%23.8) kültürlerde üreme tespit edildi. Endoftalmi tanısı konulan olguların tümünde başlangıç görme düzeyi; el hareketleri, ışık hissi ve projeksiyon yada daha kötü düzeydeydi. Altı (%28.6) olguda tedaviye 72 saat sonra yeterli cevap alınamaması üzerine ikinci kez vitrektomi yapıldı. Bir olguda postoperatif dönemde retina dekolmanı geliştiği için silikon yağı kullanılarak retina dekolmanı ameliyatı yapıldı. Bir olguda inflamasyon kontrol altına alınamadı ve eviserasyon uygulandı. Son kontrollerde 14 (%66.7) olguda görme düzeyi parmak sayma ile 0.05 arasında, 6 olguda (%28.6) ise 0.1 ve üzerinde tespit edildi. Sonuç: Katarakt cerrahisi sonrası gelişen ciddi görme kaybı olan olgularda erken dönemde yapılan 23 G PPV cerrahisi etkili ve güvenilir bir tedavi seçeneği olarak görünmektedir. Daha fazla olgu sayısı ve kontrol grubu içeren ileri çalışmalar yapılmalıdır.
