Comparison of pars plana vitrectomy and combined phacoemulsification and pars plana vitrectomy in proliferative diabetic retinopathy

Abstract

Purpose: To assess the safety and effectiveness of pars plana vitrectomy (PPV) and combined pahcoemulsification- PPV in proliferative diabetic retinopathy. Materials and Methods: Records of 64 eyes of 64 patients with type II DM that underwent PPV due to proliferative diabetic retinopathy were evaluated retrospectively. They were recruited into 2 groups: First group consisted of 29 patients (16 female, 13 male) underwent additional simultaneous phacoemulsification and intraocular lens implantation due to lens opacities causing difficulties for visualization of the posterior segment. The second group consisted of 35 patients (14 female, 21 male) that did not undergo lens extraction. Peroperative problems, complications, anatomical and functional success in both groups were compared. Results: Average follow-up time was 19±4 (mean±SEM) months in the first and 24±7 months in the second group (P>0.05). 1/10 and better visual acuity rate increased from 17% (5 eyes) to 38% (11 eyes) in the first group, from 20% (7 eyes) to 29% (10 eyes) in the second group (P>0.05). Postoperatively, eyes using topical glaucoma medicine increased from 10% (3 eyes) to 14% (4 eyes) in the first group, from 6%(2 eyes) to 9%(3 eyes) in the second group (P>0.05). Neovascular glaucoma developed in 1 (3%) eye in the first group, in 2 (6%) eyes in the second group (P>0.05). Anterior proliferation developed in 2 eyes (6%) and significant cataract in 6 eyes (17%) in the second group. Second PPV operation was applied to 2 eyes (7%) in the first, to 3 eyes (9%) in the second group (P>0.05). Six (17%) eyes had cataract operation within 1 year following the first PPV in the second group. Conclusion: Though combined surgery results in similar anatomic and functional success rate with PPV alone, combined phacoemulsification and PPV can be preferred in eyes with proliferative diabetic retinopathy and nuclear sclerosis as it prevents postoperative cataract progression.

Amaç: Proliferatif diyabetik retinopatili (PDR) gözlerde pars plana vitrektomi (PPV) uygulaması ile kombine fakoemülsifikasyon- PPV uygulamasının etkinliğini ve güvenilirliğini karşılaştırmak. Gereç ve Yöntem: PDR sebebiyle PPV endikasyonu bulunan 64 hastanın 64 gözü çalışmaya dahil edildi. Hastaların tümü tip II diyabet hastası idi. Hastalar fakoemülsifikayon cerrahisi uygulanmasına göre iki gruba ayrıldı. Birinci grup (29 hasta, 16 kadın, 13 erkek), arka segmentin görüntülenmesini güçleştirecek derecede lens kesafeti olup PPV ile aynı seansta fakoemülsifikasyon- göziçi mercek implantasyonu uygulanan hastalardan oluştu. Kristalin lensleri korunarak sadece PPV ameliyatı yapılan hastalar ise ikinci grubu (35 hasta, 14 kadın, 21 erkek) meydana getirdi. Gruplar peroperatif karşılaşılan sorunlar, komplikasyonlar, anatomik ve fonksiyonel sonuçlar bakımından kıyaslandı. Bulgular: Postoperatif takip süresi birinci grupta ortalama 19±4 ay (ortalama±SEM) , ikinci grupta 24±7 aydı (P>0.05). Birinci grupta, ameliyat öncesi 1/10 ve daha iyi olan görme keskinliği oranları %17’ten (5 göz) ameliyat sonrası son kontrolde %38’e (11 göz), ikinci grupta %20 den (7 göz) %29’ye (10 göz) çıktı (P>0.05). Glokom ilacı kullanımı birinci grupta %10’dan (3 göz) postoperatif son kontrolde %14’e (4 göz), ikinci grupta %6’dan (2 göz), %9’a (3 göz) çıktı (P>0.05). Postoperatif son kontrolde birinci grupta, 1 (%3) gözde neovasküler glokom; ikinci grupta ise 2 (%6) gözde neovasküler glokom, 2 (%6) gözde anterior proliferasyon ve 6 (%17) gözde belirgin lens kesafeti saptandı. İlk gruptaki 2 (%7), ikinci gruptaki 3 (%9) göze daha sonra ikinci PPV ameliyatı uygulandı (P>0.05). İkinci grupta postoperatif ilk bir yıl içinde 6 (%17) göze katarakt ameliyatı yapıldı. Sonuç: Kombine cerrahi yalnız başına vitrektomi uygulamasına eş değer anatomik ve fonksiyonel sonuçlar göstermekle birlike postoperatif katarakt gelişimi olasılığını önlediğinden, nükleer sklerozu bulunan PDR’li gözlerde tercih edilebilecek bir yöntemdir.
